Objective: To observe the efficacy of probiotic compound combined with enteral nutrition(EN)on chronic obstructive pulmonary disease(COPD)patients with respiratory failure.Methods: According to the criteria of COPD with respiratory failure,99 COPD patients randomly divided two groups: observation group(n=49)and control group(n=50),with random number tables.Observation group was given probiotic compound combined with EN,while the control group was given bifidobacterium powder combined with EN.The index of dyspnea,immune index,nutrition Indicators,liver and kidney function indicators,fecal ball/rod ratio,incidence of intestinal adverse events were compared.Results:(1)Two groups of patients were male 53 cases,46cases of female,mean age(74.26±8.26),the average duration(19.92±7.29)years,two groups of patients in age,sex,duration and APACHE ?score comparison difference,no statistical significance(P>0.05),comparable.(2)Both groups of patients had improved respiratory difficulty scores,and the improvement was statistically significant(P<0.05).However,the improvement rate of the observation group was better than the control group,and the difference was statistically significant(P<0.05).(3)Two groups after treatment of leukocyte count,neutrophil percentage,c-reactive protein,calcitonin original inflammatory indexes such as reduced significantly,and there is statistical significance(P<0.05),after 7 days,14 days after treatment,using composite probiotic group inflammation index falling speed,there exists significant difference with control group,and the difference was statistically significant(P<0.05).(4)There was no significant difference in serum albumin and prealbumin levels in the two groups of patients at the early stage of treatment,with no statistically significant difference(P>0.05).There was no significant difference between the treatment of serum albumin at 7 days and 14 days after treatment,and the difference was not statistically significant(P>0.05).There were significant differences between the two groups before and after the treatment of 7 days and 14 days after the treatment of albumin levels(P<0.05).(5)The percentage of lymphocytes in the observation group and control group was significantly higher than that in the two groups before treatment,and the difference was statistically significant(P<0.05).After 7 days and 14 days after intervention,the observation group of the observation group was significantly better than the control group,and the difference was statistically significant(P<0.05).(6)The comparison of blood gas analysis and the parameters of the ventilator showed significant differences between the two groups before and after treatment,and the differences were statistically significant(P<0.05),but there was no significant difference between the two groups after treatment(P>0.05).(7)Within 14 days in the treatment,The incidence of adverse events in the observation group was lower than that in the control group,and the difference between the two groups was statistically significant(P<0.05).(8)in this experiment,liver and kidney function were in normal range,after repeated measurement,the observation group and control group of ALT,AST,BUN,Cr and so on all is normal,in the treatment of 1 day,7 days,14 days,before and after the group more and more had no obvious difference between groups,there was no statistically significant difference(P>0.05).(9)The excrement ball/pole ratio result showed that,the observation group coccus ?40% 8 examples(16.3%),the control group coccus ?40% 24 examples(48.0%),two group of between differences have statistics significance(P < 0.05).Conclusion: Compound probiotics joint enteral nutrition in the treatment of COPD with respiratory failure patients,can be effective to improve the patient's clinical symptoms,nutritional status,level of inflammation in the body and the intestinal adverse events,adjust the ecological environment and recovery in patients with gastrointestinal tract flora,comprehensive improve the treatment efficacy of combination of respiratory failure in patients with COPD.
