Objective:In the study,we group patients with respiratory failure who have invasive mechanical ventilation in accordance with TCM syndrome differentiation. Then we analyze and compare the severity of the disease, the nutritional risk and the incidence of gastrointestinal dysfunction in patients with different syndrome types. Our aim is to identify patients who are prone to malnutrition and gastrointestinal dysfunction early in the clinical work, and to develop a more reasonable enteral nutrition support program, and to treat them as early as possible to prevent gastrointestinal dysfunction. Methods:In this study, the main conditions for our collection of patients are in accordance with the diagnosis of respiratory failure and invasive mechanical ventilation, and they also received enteral nutrition treatment for ≥7 days, collection time is from February 2016 to February 2019.Then we group patients according to TCM syndromes. We complete the APACHE II score,SOFA score,NRS 2002 score, and NUTRIC score 24 hours after the patient entered the ICU,and collect albumin, prealbumin, hemoglobin, and lymphocyte counts when the patient enter the ICU.At the same time,we score gastrointestinal function from the first day of the patient receiving enteral nutrition therapy for seven consecutive days. Finally,we use SPSS software to analyze data. Results:We find significant differences in the APACHE II score,SOFA score,NUTRIC score, lymphocyte count, and prealbumin in the three groups. We further analyze these patients and find that the third group of patients have significantly higher APACHE Ⅱ scores and SOFA scores than the previous two groups. At the same time, the NUTRIC score of the third group is significantly higher than the other two groups, and the lymphocyte count and prealbumin are lower than the other two groups. The incidence of gastrointestinal dysfunction in the third group is also higher than in the other groups. Conclusion:Through the study,we draw the following conclusions:1.The third group of patients is more critical than the other two groups, and the perfusion of each organ is prominent;2.The NUTRIC score is more helpful than the NRS 2002 score to screen the nutritional risk of patients with respiratory failure;3.Lymphocyte count and prealbumin are more specific for assessing the nutritional status of patients with respiratory failure;4.The third group of patients has a higher risk of malnutrition and is more prone to gastrointestinal dysfunction,we need to give these patients more energy and pay attention to prevent the occurrence of gastrointestinal dysfunction.
