Early Diagnosis Of Sepsis By Serum Presepsin

BackgroundThe consensus of treating sepsis is that early recognition and to give effective anti-infection treatment.As a result,some new biomarkers that can be accurate and rapid diagnosis of sepsis are in urgent need of discovery.Presepsin(sCD14-ST)was discovered in 2004 and it is a kind of novel inflammatory factor.As a potential diagnostic value of experimental biomarker,it also has aroused highly valued in the process of sepsisObjectiveTo reasearch Presespsin roles in sepsis and to reduce the fatality rate.MethodCollecting 55 cases from the First affiliated hospital of Soochow University infectious diseases department from November 2015 to November 2016.A group is normal people as control group.another group is experimental group.It includes two groups of patients with local infection and sepsis patients.All people were drawn venous blood in the hospital admission on the same day or the next day.Testing results of the analysis and comparison between groups of Presepsin,PCT,the WBC,Neu%,Lym% in the distribution of each group..Experimental results were processed STATA12 package.Using the chi-square test methods for testing.Calculating the specificity,sensitivity,likelihood ratio,cutpoint and area under the ROC curve(AUC).Result1 To compare the experimental group and the control group of Presepsin,PCT,WBC,Neu% and Lym%,the results of experimental group are 587.8 ± 182.9pg/ml,3.54±8.46ng/ml,10.66±5.91,0.78±0.11,0.14±0.12 and the results of control group are 237.0 ± 27.1pg/ml,0.15±0.04ng/ml,6.12±1.11,0.46±0.11,0.28±0.65.p values are 0.001,0.03,0.0001,0.0001.2.To compare the sepsis group and the local infection group of Presepsin,PCT,WBC,Neu%,Lym%,the Presepsin,PCT of sepsis group are 698.51±166.04pg/ml,7.25±11.11ng/ml and the Presepsin,PCT of the local infection group are 471.07±133.88 pg/ml,1.09±1.27ng/ml.P values are 0.001 and 0.002.They have statistically significant.The WBC,Neu%,Lym% of sepsis group are 12.29±5.25,0.82 ± 0.09,0.09 ± 0.07.The WBC,Neu%,Lym% of local infection group are 10.38±3.09,0.78±0.09,0.15±0.09.P values are 0.09,0.06,0.07.That shows no statistical significance.3.The Presepsin,PCT,WBC,Neu %,and Lym % of specificity are 78.1%,68.7% 75.0%,62.5%,43.7%and that sensitivity are 73.9%,78.2%,60.9%,69.6%,52.2%and that AUC are 0.854,0.805,0.615,0.651,0.328.Conclusion1.Presepsin can be effective in the diagnosis of early sepsis.Its cutpoint is 587.7 pg/ml.2.Presepsin more efficient than PCT in diagnosis of sepsis.3.Presepsin can effectively distinguish between local infection and sepsis..
