Objective:To discuss the role of plamsa neutrophil gelatinase-associated lipocalin(NGAL)in the assessment of renal injury in patients with hypertension and type 2 diabetes.Methods:Collected general information and plasma samples of 996 physical health examination adults,plasma NGAL values were determined by double-antibody sandwich enzyme-linked immunosorbent assay and analyzed the influence factors of its expression.Subgroup the influenced diseases and analysed the correlation between plasma NGAL and eGFR,85 patients with hypertension and 35 cases of type 2 diabetes,Calculated the eGFR by used Cockcroft-Gault formula and divided into three groups by eGFR.1)renal function normal control group:eGFR>90 mL/min/1.73 m2(2)mild damage to kidney function groups:eGFR 60-90 mL/min/1.73 m2(3)moderately impaired renal function groups:eGFR 30-60 mL/min/1,73m2.Then analyzed the relationship between plasma NGAL and renal injury in hypertensive and 2 diabetic patients.Results:(1)Pearson correlation analysis showed that plasma NGAL value was positively correlated with age?fasting plasma glucose?serum creatinine?endogenous creatinine clearance rate(r=0.213?0.094?0.106,P<0.05)and negative correlation with eGFR(r=-0.134,P<0.05)(2)The expressions of plasma NGAL also have significant differences in adults with hypertension?type 2 diabetes and dyslipidemia(P<0.05)when compared with the corresponding normal groups(P<0.05).(3)In patients with hypertension and type 2 diabetes,the value of plasma NGAL with mild and moderate renal impairment was significantly higher than that in the normal control group(P<0.05).(4)The ROC curve analysis showed that when plasma NGAL was diagnosing renal injury in hypertensive patients,it's sensitivity rate was 68.75%,specificity rate was 62.12%and AUC area was 0.7517.When plasma NGAL was diagnosing renal injury in patients with type 2 diabetes,it's sensitivity rate was 81.82%,specificity rate was 53.85%and AUC area was 0.7431.It is suggested that plasma NGAL has a higher sensitivity in the diagnosis of kidney injury in patients with type 2 diabetes.Conclusion:The expression of plasma NGAL had negative correlation with eGFR,and was significantly higher in people aged>65 years.Plasma NGAL value increased significantly in adults with hypertension and type 2 diabetes.It was superior to traditional indicator blood creatinine and urine protein when evaluating kidney function e.It was expected to be a predictor factor of renal injury and increase the detection rate of kidney injury in the patients who have the high risk diseases.
