The Changes Of Serum Vit D And DPP-4 Levels In Patients With Obstructive Sleep Apnea Hypopnea Syndrome

In this experiment,we compared the correlations between the serum Vit D levels and the serum DPP-4 levels and above indexes and investigated the changes of serum Vit D and DPP-4 levels in patients with OSAHS by measuring the serum levels of vitamin D(Vit D)and dipeptidyl peptidase-4(DPP-4)before and after sleep,sleep apnea hypopnea index(AHI),body mass index(BMI),neck circumference,waist hip ratio,body fat percentage,the lowest arterial oxygen saturation(LSa02),the mean arterial oxygen saturation(MSaO2)and other indicators of different severity of obstructive sleep apnea hypopnea syndrome(OSAHS)and non-OSAHS subjects.Objective:To study the changes of serum Vit D and DPP-4 levels in patients with different severity of OSAHS before and after sleep,and to explore the relationship between Vit D,DPP-4 and OSAHS.Methods:The experiment included 80 subjects who received polysomnography and met the inclusion criteria during the period from February 2014 to February 2015 at the sleep monitoring center of the respiratory medicine department of our hospital.Because the clinical symptoms of mild OSASH patients and moderate OSASH patients were lighter than the severe OSASH patients,and patients with mild OSASH and moderate OSASH coming to the hospital were relatively less than the patients with severe OSASH.In preliminary test,there was no difference between the mild OSASH subjects and moderate OSASH subjects in the serum levels of Vit D and DPP-4.Thus,we combined mild and moderate OSASH subjects into one subgroup,namely non-severe OSAHS group.All subjects were divided into three groups,as normal control group(23 cases),non-severe OSAHS group(23 cases),severe OSAHS group(34 cases).The serum levels of Vit D(ng/ml)and DPP-4(ng/ml)were tested by using the method of enzyme linked immunosorbent assay and analyzed statistically.Results:1 The serum Vit D levels:In normal control group,non-severe OSAHS group and severe OSAHS group,the serum Vit D levels after sleep were higher than before sleep(P<0.05).There was no difference among the subjects in each group in serum Vit D levels before sleep(P>0.05).The Vit D levels after sleep of severe OSAHS group are lower than that of the normal control group and non-severe OSAHS group(P<0.05).The Vit D levels after sleep of non-severe OSAHS group are lower than that of the normal control group(P<0.05).2 The serum DPP-4 levels:In normal control group and non-severe OSAHS group,the serum DPP-4 levels before and after sleep showed no difference(P>0.05).In the severe OSAHS group,the serum level of DPP-4 after sleep was lower than that of before sleep(P<0.05).There was no difference among the subjects in each group in serum DPP-4 levels before sleep(P>0.05).In severe OSAHS group,the serum level of DPP-4 after sleep was lower than that of in non-severe OSAHS group(P<0.05).There were no differences between the control group and the severe OSAHS group and severe OSAHS groups in serum DPP-4 levels after sleep(P>0.05).3 Pearson Correlation analysis:In OSAHS group,there was no correlation between the serum Vit D and DPP-4 level before sleep and BMI,AHI,neck circumference,waist hip ratio,body fat percentage,LSa02 and MSa02(P>0.05).The serum level of Vit D after sleep in the OSAHS group was negatively correlated with AHI(P<0.05).The serum level of DPP-4 after sleep in the OSAHS group was negatively correlated with AHI(P<0.05).In OSAHS group,there were no correlation between the serum Vit D and DPP-4 levels after sleep and BMI,neck circumference,waist hip ratio,body fat percentage,LSaO2 and MSaO2(P>0.05).In OSAHS group,AHI was positively associated with BMI(P<0.05).AHI showed negatively correlated with LSaO2and MSaO2 in OSAHS group(P<0.05).AHI and neck circumference,waist hip ratio,body fat percentage had no correlation in OSAHS group(P>0.05).In OSAHS group,BMI showed negatively correlated with LSaO2 and MSaO2(P<0.05).The OSAHS group of subjects had no correlation between BMI and neck circumference,waist hip ratio,body fat percentage(P>0.05).Conclusion:1 Whether or not suffering from OSAHS,the serum Vit D levels after sleep are higher than before sleep.2 In OSAHS groups,the serum levels of Vit D after sleep are negatively correlated with AHI,and the more severe the OSAHS,the lower the serum Vit D level.3 The serum DPP-4 levels in patients with severe OSAHS after sleep are lower than that before sleep.4 In OSAHS groups,the level of serum Vit D after sleep shows negative correlated with AHI,the lower the level of OSAHS,the lower the level of serum DPP-4.
