Objective:According to the collection and analysis of clinical retrospective of inpatients with adult iron-deficiency anemia in class 3A hospital of Qiqihar during 2014-2017,we summarized the demographic characteristics,clinical symptoms,laboratory examination,etiology of iron-deficiency anemia in this region,focusing on discussing the features of the etilogy and laboratory examination of iron-deficiency anemia,aiming at providing a reference for clinical diagnosis and treatment of iron-deficiency anemia in this region.Method:494 cases of inpatients with adult iron-deficiency anemia in class 3A hospital of Qiqihar from April 2014 to April 2017 were selected.The clinical data of patients were retrospectively analyzed(including patient demographic characteristics,clinical symptoms,etiology,laboratory examination,etc.),and were established data base by using software Epidata 3.0,and statistical software SPSS 20.0 data for statistical analysis.Results:1.In this survey,there were in total 494 cases,including 134 males(27.1%)and 360 females(72.9%),among these cases,there were 171 cases(34.6%)under 44 years old,188 cases(38.1%)at 45-59 years old,135 cases(27.3%)above 60 years old.There were 299 urban residents(60.5%),195 rural residents(39.5%).There were 299 cases(60.5%)of moderate anemia,which is the largest proportion of anemia,100 cases(20.2%)of severe anemia and 95 cases(19.2%)of mild anemia.2.Clinical manifestations of the Iron-deficiency primary disease,the main sympotems in male was abdominal discomfort and in female was menorrhagia,there were 78 cases(58.2%)and 153 cases(42.5%),respectively;for common symptoms of iron-deficiency,dizziness and weak were the main clinical manifestations(93.3% and 92.8%);for the systemic iron-deficiency both male and female were given priority to the Spirit abnormal behavior(48.5% and 51.4%).3.The etiology of male patients was mainly gastrointestinal inflammation and ulcer,with 56 cases(41.8%).For female,menorrhagia was the main cause,with 152 cases(42.5%).The proportion of middle-aged and elderly men who have anemia due to gastrointestinal tumor and digestive tract polyps was higher than that of young men.The proportion of middle-aged and elderly men who have anemia due to gastrointestinal tumors and gastrointestinal polyp was higher than that of young men.The proportion of middle-aged and elderly women who have anemia due to gastrointestinal tumors was higher than that of young women.4.In peripheral blood index,the detection rate of platelet count was 55.3%.The mean volume of red blood cells was negatively correlated with the number of platelets(r=-0.211,P< 0.001);the detection rate of erythropoietin increase was 99.4%,and erythropoietin was negatively correlated with hemoglobin(r =-0.173,P< 0.001).5.For myelogram,the detection rates of the megakaryocyte increased was 89.1%,and the myeloproliferative degree that are significantly active was 34.3%,which is on the rise with the increasing of anemia degree(P<0.05)by the bone marrow smear examination.Conclusions:1.Among the subjects,the proportion of female patients with Iron-deficiency Anemia was higher than that of the male patients,and the female patients were mainly caused by the increase of menstruation.It is suggested that the health education of women of childbearing age and the health examination of the reproductive system should be strengthened so as to prevent the occurrence of Iron-deficiency Anemia and the early treatment of the original disease.2.For adult male and non-menstruation female,the main etiology were gastrointestinal inflammation,ulcers and gastrointestinal tumors,suggesting that the health examination of gastrocolonoscopys is an important tool to confirm the etiology of anemia.3.The detection rates of the myeloproliferative significantly active degree was showed an upward trend with the increase of anemia,which remaindered that doctors should pay close attention to changes of indictors when the patients appeared anemia,bone marrow activity compensatory increases.4.The detection rate of thrombocytopenia increased.The mean volume of red blood cells was negatively correlated with the number of platelets.Clinicians should identify the cause of thrombocytosis in patients with Iron-deficiency Anemia.5.The detection rate of erythropoietin increased by more than 99%.The erythropoietin value is negatively correlated with the amount of hemoglobin.Monitoring the level of erythropoietin can provide evidence for the diagnosis of Iron-deficiency Anemia.
