Objective: After the implementation of the second child policy in China, the number of pregnant women is increasing.During the whole pregnancy, various complications during pregnancy will occur.Anemia is a common complication during pregnancy.Anemia during pregnancy is a high-risk pregnancy [1.2].According to the World Health Organization(WHO),the incidence of anemia among pregnant women, especially in Asian countries,is about 50%.Although China's economy is developing rapidly, this does not mean that the prevalence of anemia in China will be reduced. In 2016,the literature on the prevalence of iron deficiency anemia in pregnant women in China pointed out that iron deficiency in East China and Northeast China The standardized prevalence rate was relatively high,57.37% and 53.41%,respectively,while the Southwest was the lowest,30.51%.The prevalence of standardized iron deficiency anemia in the south-central,north-western and eastern regions is 21.30%,16.97% and 17.53%,respectively.Therefore,anemia during pregnancy is still a problem that cannot be ignored,so in order to further strengthen maternal Systematic management, through the investigation of serum ferritin and hemoglobin levels during pregnancy, timely detection and treatment of adverse factors, provide reasonable guidance during pregnancy, reduce the incidence of iron deficiency anemia during pregnancy, thereby reducing the prevalence of pregnant women and perinatal Mortality, this paper studied the prevalence of iron deficiency anemia and iron deficiency anemia in pregnant women in Dalian.Method: Pregnant women who were routinely examined at the Second Affiliated Hospital of Dalian Medical University and Dalian Maternity Hospital from August 1st,2018 to January 31 st,2019.There are relatively complete inspection data.Remove other causes of anemia, such as: autoimmune hemolytic disease, aplastic anemia, abnormal bone marrow hyperplasia, rheumatic immune disease, peptic ulcer, gastrointestinal dysfunction, kidney disease, thalassemia, giant cell anemia, cancer Diseases such as anemia, complications such as prenatal bleeding. However, pregnant women who are supplemented with iron during pregnancy are not excluded.A total of 1562 pregnant women with complete data were obtained. All pregnant women were treated with elbow venous blood during pregnancy to detect serum ferritin and hemoglobin.Blood routine examinations were automatically performed using a fully automated blood analyzer and serum ferritin was detected using a chemiluminescent microparticle immunoassay.1 The age of pregnant women is 20-25 years old,26-30 years old,31-35 years old,36-40 years old,41 years old or older: Education level: undergraduate and above, junior college, high school and secondary school, junior high school and below;maternity history Primipara,maternal.2 According to the values of maternal serum ferritin and hemoglobin,it is divided into mild, moderate, severe and extremely severe anemia, and the light, medium, heavy, extremely severe anemia rate and anemia rate during pregnancy are calculated respectively.3 All maternal women were divided into 5 groups according to different gestational weeks, which were <14,14 ? gestational weeks <21,21 ? gestational weeks <28,28 ? gestational weeks <37,37 to labor, respectively,to study iron deficiency and The incidence of iron deficiency anemia.4 All pregnant women were divided into 4 groups according to different serum ferritin levels: <10ng/ml,10-19.9 ng/ml,20-29.9 ng/ml,?30 ng/ml,studied during pregnancy.Iron deficiency and iron deficiency anemia,ie early pregnancy(? 14 weeks),mid-pregnancy(15 ? gestational weeks < 28)and late pregnancy(28 ? gestational weeks-labor)anemia incidence according to hemoglobin The value of all pregnant women were divided into normal group(hemoglobin ? 110 g / l)and anemia group(hemoglobin < 110 g / l),respectively,to study the relationship between weight gain during pregnancy and anemia, that is,weight gain during pregnancy <10kg,weight gain during pregnancy It is the incidence of anemia of 10kg-15 kg,weight gain during pregnancy,16kg-20 kg,and weight gain during pregnancy ?20kg.Statistical studies were performed using SPSS 21.0 software, and there was statistical significance at P < 0.05.Result: 1.Study the general condition of the subject.During the study,1,562 pregnant women met the study criteria and had well-documented data for analysis. The average age is(24.16 ± 5.17)years old,of which 20-25 years old: 13.51%(212/1562),26-30 years old: 43.46%(679/1562),31-35 years old: 26.37%(412/1562),36-40 years old: 13.44%(210/1562),over 41 years old: 3.13%(49/1562);education level: undergraduate and above 45.77%(715/1562),junior college, high school and technical secondary school : 33.48%(523/1562),junior high school and below: 20.74%(324/1562);1028 cases of primipara, accounting for 65.8%(1028/1562),534 cases of maternal, accounting for 34.1%(534/1562).The prevalence of anemia and iron deficiency was not significantly different among pregnant women of different ages, cultures, and births.2.Anemia during pregnancy. The prevalence of anemia during pregnancy was 52.24%(816/1562),and the incidence of anemia was still high. The incidence of mild and moderate anemia is 50.38%,2.43%,respectively,and does not have severe and extremely severe anemia, mainly mild anemia.3.Iron deficiency and iron deficiency anemia occur in each gestational age.All maternal women were divided into 5 groups according to different gestational weeks, which were <14,14 ? gestational weeks < 21,21 ? gestational weeks < 28,28 ? gestational weeks <37,37 to labor, the incidence of iron deficiency was : 19.08%,23.92%,45.48%,51.01%,53.78%,the incidence of iron deficiency anemia were: 3.81%,9.56%,25.54%,30.89%,38.87%.During 21 ? gestational weeks <28,the incidence of iron deficiency and iron deficiency anemia was 45.48% and 25.54%,respectively.Compared with the prevalence of gestational age,the incidence of iron deficiency and iron deficiency anemia was significant.Increased, through analysis,we can tell that there is a statistically significant difference in maternal iron deficiency between different gestational weeks(X2=592.3102 P<.0001).4.Distribution of anemia in different SF values during each pregnancy. All pregnant women were divided into 4 groups according to different serum ferritin levels: <10ng/ml,10-19.9 ng/ml,20-29.9 ng/ml,?30 ng/ml,study the anemia during pregnancy The prevalence of anemia in early pregnancy(? 14 weeks)was 6.84%,3.60%,2.52%,and 0.87%,respectively.The incidence of anemia in mid-pregnancy(15 ? gestational weeks < 28 weeks)was 37.16%.,25.47%,27.43%,2.96%;the incidence of anemia in late pregnancy(? 28 weeks-labor)was 50.67%,45.14%,35.37%,4.53%.It can be seen from the analysis that there is a statistically significant difference in the incidence of anemia between different SF groups(X2=593.6335 P<.0001).5.The relationship between weight gain during pregnancy and anemia.According to the value of hemoglobin, all pregnant women were divided into normal group(hemoglobin ? 110 g / l)and anemia group(hemoglobin < 110 g / l),respectively,to study the relationship between weight gain during pregnancy and anemia, including weight gain during pregnancy <10kg anemia The incidence rate was 14.8%,the prevalence of anemia of 10kg-15 kg during pregnancy was 31.61%,the incidence of anemia of 16kg-20 kg during pregnancy was 37.6%,and the incidence of anemia of ?20kg during pregnancy was 18.57%.The analysis showed that there was a statistically significant difference in the weight gain during pregnancy between the normal group and the anemia group(X2 = 46.3852 P <.0001).Conclusion: 1.The incidence of iron deficiency anemia is significantly increased from 21 weeks to 28 weeks of pregnancy. It is recommended to start an appropriate amount of iron supplementation at 21 weeks of pregnancy.2.During pregnancy,we should attach great importance to the value of SF,provide reasonable guidance during pregnancy, reduce the incidence of iron deficiency anemia during pregnancy, thereby reducing the prevalence and mortality of pregnant women and perinatal, blood serum combined with serum ferritin detection during pregnancy It is of great clinical significance for the prevention and treatment of iron deficiency anemia during pregnancy.3.It is recommended to manage weight properly during pregnancy to avoid excessive weight gain and maintain good eating habits.
