Objective The research applied Comprehensive Geriatric Assessment(CGA)to the clinical diagnosis and treatment of elderly hospitalized hypertensive patients to investigate the occurrence of anxiety and depression,and explored the effects of CGA and intervention on anxiety,depression,and blood pressure in hypertensive patients.The purpose is to provide evidence for improving the diagnosis and treatment of chronic diseases and improving the quality of life in aged patients.Method To select elderly patients with hypertension in our department of geriatrics for the study randomly.Comprehensive geriatric assessment and general data investigation were performed on the patients enrolled in the study.1 The Hamilton Anxiety Scale(Hamilton Anxiety Scale,HAMA)and the self rating Depression Scale(Self-rating Depression Scale,SDS)were used to evaluate the emotional status of the anxiety or depression group and the non anxiety depression group.2 Hypertensive patients with anxiety or depression were randomly divided into CGA group and psychological group.No anxiety or depression were set as control group.The CGA group performed individualized interventions on the basis of the assessment,the psychological group performed conventional antihypertensive therapy and psychological counseling,and the control group received conventional antihypertensive therapy.After 3 weeks,the indexes such as CGA and blood pressure were reviewed.Results 1 A total of 317 effective cases were collected in this study,including 173 males and 144 females,aged 65 to 96 years,86(27.1%)with anxiety,and 91(28.7%)with depression.2 There were significant differences between 24 hours of mean blood pressure,barthel activities of daily living(BADL)and sleep quality in the anxiety group and the anxious group(P<0.05);There was significant difference in the mean blood pressure,BADL,cognitive function and sleep quality between the two groups in depression group and non-depression group(P<0.05).3 The blood pressure,anxiety and depression control of the three groups were improved(P<0.05),and the CGA group had better intervention than the psychological intervention group and the control group,with significant differences.(P<0.01).Conclusions Hypertension is associated with high incidence of anxiety and depression.Female,high blood pressure,cognitive dysfunction,disability and sleep disorders are independent risk factors for hypertension associated with anxiety and depression.Using CGA can can detect the potential psychological problems in early stage,and implement personalized intervention treatment timely,so as to improve the diagnosis and treatment effect and improve the quality of life of patients.
