The Study Of Public Hospital Reform Concentrating On Cancellation Of Medicine Markups

Since the reform and opening-up,Chinese health service sector has developed greatly.However,the problem of high cost and difficulty of getting medical treatment became more and more obvious.How to solve this problem? The reason why it is so expensive to obtain medical service mostly depended on covering hospital expenses with drug sales.It was born in the age of planned economy.At that time,no big problem was induced under the traditional national health care system.With the advance of reform and opening-up in China,the above compensation system triggered a series of problems,such as non-standard practice of diagnosis and treatment,repeated check and excessive prescriptions.And doctors' commercial bribery in health service sector existed for a long time.The drug price remained artificially high.Thus,the relationship between doctor and patients become more and more conflicted since the beginning of the new century.The high cost of medical service attributed to not only drug manufacturers,but also the premium in the drugs circulation process.In addition,to maintain the regular operation,public hospitals attempted to chase for profit of expensive drugs under the background of few government regulation.Therefore,the central government started to publish and conduct a series of health care reform policy.It was the important character to join reformation for health services,medical insurance,and medicine production-circulation.Sanming City and Beijing medical administration reform provided us a good example.The success of Sanming medical reform was reflected in following three aspects.Firstly,the medical burden of patients reduced greatly.Secondly,the hospitals' actual income increased greatly.Thirdly,health insurance pooling fund made a profit instead of suffering a loss.Sanming medical reform presented good experience and reference for advance of Chinese medical reform.In order to solve the problem of difficulties and expensive medical treatment for the general people,cancellation of medicine markups in public hospital was a very vital part.In the base of “Guiding opinions on the pilot reform of public hospitals”published by five ministries and commissions in 2010,the reform goal of compensation mechanism of public hospitals was to promote separation of clinic from pharmacy and break mechanism of supplement clinic profit from drugs.The formation of drug price addition had many reasons.Firstly,the health service sector always had not enough fund from the government since the reform and opening-up.Secondly,thespectrum of disease in China has changed significantly in recent years.Thirdly,the price of medical service would not presented labor value of doctors,result in excessive prescriptions.Fourthly,the benefit chain lied in the process of pharmaceutical production and circulation.Fifthly,the current medical insurance payment system had imperfection and could not kept up with the medical situations in China.We compared health reform in Sanming City and Beijing,no matter what patterns,the compensation for public hospital was consisted of medical service profits and public subsidies after cancelling medicine markups.The cancer hospital,as a typical specialized hospital,the profits of medicine markups accounted much more than general hospital.The cancer hospital faced more financial pressure.Through typical survey,we have acquired and analyzed related data of JiangXi Cancer Hospital.After cancelling medicine markups,we could make up financial gap through the following three aspects.Firstly,adjust the price of various medical service,for example registration and treatment fee,consultation fee,pathological and cellula diagnosis fee,radiation therapy fee,multiple disciplinary diagnosis fee and so on.Secondly,increase government subsidies and public funds.Thirdly,improve working efficiency of hospital operation,reduce circulation cost and reform the patterns of medicare insurance payments.
