| Objective1.Research on the basic psychological needs scale for assisted reproductive technology patients(BPNART),to provide appropriate evaluation tools for medical staff.2.The basic psychological needs of the patients with assisted reproductive technology were investigated by using compiledthe scale and the influencing factors were analyzed to provide theoretical basis for clinical medical staff to formulate individualized psychological intervention.Methods1.A qualitative research was conducted in the first phase.Form the semi-structured interview outline based on the literature,books,and discussion of team members.Using the method of purposive sampling,6 medical staff of reproductive department and 10 infertility patients were selected from the reproductive center hospitals in Nanchang.They were interviewed on the basis of the interview outline,to understand the basic psychological needs of assisted reproductive technology patients.The phenomenological analysis method is used to summarize and organize the data.2.The development of the scale and the testing of reliability and validity was implemented in the second phase.Based on literature review and qualitative interview results,the dimensions and item pool of basic psychological needs questionnaire for patients with assisted reproductive technology were constructed.Using a purposive sampling method,the field of clinical and nursing experts,psychologists,statistics,a total of 21 were selected for two rounds of Delphi expert consultation.They further selection and supplement the preliminary items,delete irrelevant or unimportant items,combine items with similar meanings,and initially formed the basic psychological needs scale for patients with assisted reproductive technology.Using the method of cluster sampling,a preliminary questionnaire was used to investigate300 infertile patients who entered the ART treatment cycle from all the hospitals set up reproductive medicine in Nanchang.Using Cronbach,s coefficient,Split-half reliability and test-retest reliability to assess questionnaire reliability;Structure validity,content validity to evaluate questionnaire validity.Finally,the basic psychological needs scale of the patients with assisted reproductive technology was formed.3.The third stage:the survey of basic psychological needs of patients with assisted reproductive technology.Using cluster sampling method,240 infertile patients who entered the ART treatment cycle were selected from all hospitals in Nanchang City where the reproductive medicine was set up.Using the self-designed general data questionnaire and self-made assisted reproductive technology patients'basic psychological needs scale were used to investigate present situation and analyse influence factors.4.Statistical methods:The data of this study were recorded by Excel2010 and analyzed statistically by SPSS22.0 and AMOS22.0 statistical software.We use mean±standard deviation,frequency,composition ratio to describe data.The two independent samples t test,one-way ANOVA,multiple linear regression analysis were used to evaluate the influencing factors of infertile patients'level of BPNART.All tests were two-sided test,with P<0.05 as the difference was statistically significant.Results1.The first phase:qualitative research.Qualitative research results:From the point of view of medical staffs and patients,we further understand the basic psychological needs of the patients with assisted reproductive technology.Qualitative research extracted 3 dimensions,52entries,including independent need dimensions of the 15 items,16 items in the ability needs dimension,relationship needs dimension 21 items.2.The second phase:scale development and reliability and validity test.(1)Expert consultation's result:2 rounds of experts enquiry send out 42questionnaire,so expert positive coefficients were 100%and authority coefficient of experts were 0.88,0.90.The range of variation coefficient was 0.08~0.19 and coordination coefficient was 0.12~0.37.After chi square test,the difference was statistically significant(P<0.05).Suggesting expert coordination degree is better.The basic psychological needs scale of patients undergoing assisted reproductive technology has 3 primary indexes,6 secondary indexes and 36 three-level indexes.(2)The result of the reliability and validity of the questionnaire:(1)With the item screening and exploratory factor analysis(EFA):Exploratory factor analysis resulted that the KMO=0.890,meanwhile,the Bartlett spherical test of?~2 value was9442.893,it has Significance statistical(P=0.000,P<0.01).This scale is suitable for factor analysis.Using the principal component analysis to extract the common factor,take the characteristic root>l,it produced 3 common factor,cumulative variance contribution rate was 79.935%.The factor load of each item in the independent demand dimension was 0.759~0.815,and that in the ability requirement dimension was 0.642~0.801.The payload of each item in the relational requirement dimension was 0.571~0.821.(2)With the Confirmatory factor analysis(CFA):(?~2/df)=1.674,CFI=0.902,GFI=0.76,IFI=0.90,TLI=0.900,RMR=0.029 and RMSEA=0.065,indicating that the model fitting is better.(3)Content validity analysis:In the first round,experts evaluated the validity of 52 items,the Item-Content Validity Index(I-CVI)ranged from 0.520~1.000,the Scale-Content Validity Index(S-CVI)was 0.870.Round 2 expert experts estimated the validity of36 items,the Item-Content Validity Index(I-CVI)ranged from?0.900~1.000,the Scale-Content Validity Index(S-CVI)was0.980.(4)The analysis of the reliability of the scale:Total scale Cronbanch's alpha was 0.947,each domain Cronbanch's alpha coefficient range0.889~0.927.Total questionnaire a half reliability was 0.902,all dimensions a half reliability coefficient range 0.887~0.911.The test-retest reliability was 0.952 for the whole scale,and 0.858~0.912 for three dimensions?3.The third stage:the survey of basic psychological needs of patients with assisted reproductive technology.The total score of basic psychological needs of infertile patients was(143.34±11.48),and the highest score was the dimension of ability needs(4.07±0.41),and the lowest was independent demand dimension(3.83±0.39).Multiple linear stepwise regression analysis showed that the main influencing factors of infertile patients were education level,infertility duration,and treatment cycle.Conclusions1.The basic psychological needs of the patients with assisted reproductive technology developed in this study has good reliability and validity and can objectively reflect the present situation of the basic psychological needs of infertile patients.2.The basic psychological needs of infertile patients are higher.Moreover,the education level,infertility duration and treatment cycle are the main factors affecting the basic psychological needs of infertility patients.It is suggested that medical staff can provide individualized psychological nursing for infertile patients in order to relieve their psychological burden and improve their quality of life. |
