Objective: This project by building a model of venous crisis after replanting completely amputated rabbit ear,and using the improvement of vacuum sealing drainage(VSD)device for treating venous crisis of rabbit ear,to scientifically evaluate the influence and mechanism of VSD device to venous crisis,and explore the feasibility of its application in clinical medicine.Methods: First randomly took 10 experimental rabbit to make models,each rabbit ears were randomly choose for the VSD treatment group and the control group respectively.And then treated with improved VSD device for venous crisis of rabbit ear.Through the observation and then recorded the color,skin temperature,blood capillary reaction and swelling degree of rabbit ears,to help assess the VSD for relief of venous crisis situation.Representative point of rabbit ear specimens were collected for pathological biopsy specimens,and each time point with VEGF(vascular endothelial growth factor)and CD34 of immunohistochemical analysis,evaluated angiogenesis in the process of relieving venous crisis,to screen more important promotingangiogenesis factor.Finally measured survival area of replantation rabbit ear,judged in venous crisis after replantation of rabbit ear of the whole process of prognosis.The experimental data were collected for statistical analysis.Results:1.Postoperative experimental animals in general observation: the VSD treatment group compared with own control group,replantation in rabbit ears respectively 1,3,5,7,14 days in turn from its color,blood capillary reaction,skin temperature and swelling degree had obvious differences,the treatment group was better than the control group.2.Histological manifestations: after operation in 1,3,5,7,14 days,HE dyeing observation found that both the treatment group and control group had different degrees of inflammatory cells infiltrated,erythrocyte sedimentation,tissue edema,and correspondingly the VSD treatment group was significantly lighter than the control group.3.Each time point of postoperation,both the VSD group and its own control group were expressed with VEGF and CD34,illustrated important role of them in angiogenesis.However,VEGF marked microvascular density(MVD)is significantly higher than that of the CD34 markers,which prompted VEGF may be the main factor in the process of promoting angiogenesis of VSD treatment.4.Two weeks of postoperation,the final survival area of replantated rabbit ear: the VSD group was 36±3cm2,control group was 4±4cm2,P <0.01 and had statistically significant,notable differences in the two groups:most part of the tissue of treatment group was survive while most part of the control group was necrosis.Conclusion: The vacuum sealing drainage(VSD)can effectively relieve venous crisis after replantation in a model of rabbit ear,and the survival rate significantly higher than the conventional bloodletting therapy.The mechanism,on the one hand,is that mechanical drainage effect improves microcirculation,on the other hand,it promotes the new capillary.And speculating that VEGF is a dominant role in regulating angiogenesis by treating with VSD.
