Surgical Treatment Of AC Joint Dislocation:Double Endobutton Vs Clavicular Hook Plate Technique

BACKGROUND:Various surgical techniques have beeen used in the treatment of acromioclavicular joint dislocation.Whereas the optimal technique is unclear.In the present study,we aimed to compare clinical outcome between hook plate fixation and double Endobutton technique for the treatment of acromioclavicular(AC)joint dislocation.OBJECTIVE:The purpose of this study is to compare the surgical results of ACJ dislocation treated either by double Endobutton technique or clavicular hook plate technique.METHODS:A retrospective study was conducted for patients who underwent surgical treatment for ACJ dislocation from January 2013-February 2016 in Zhong Da Hospital affiliated to Southeast University.Overall 34 patients were included and divided into two groups on the basis of surgical techniques:Hook plate fixation(group I,20 cases)and double Endobutton technique(group ?,14 cases).the time point for follow up were 12-16 month of post operation.Clinical outcomes were evaluated by constant-murley score system for shoulder function and visual analog score(VAS)for pain.The hospital stay,duration of surgery,blood loss during surgery and postoperative complications were also recorded.All values are expressed as their Mean±Standard Deviation(SD).SPSS v.20 was used for statistical analysis.Either a T-test or Non parametric Mann-whitney test was used to compare variables between groups.RESULTS:The mean Constant-Murley score at the final follow up was 80.5± 9.7 in groups?I which was significantly lower than that(92.2±7.2)in group ?(p<0.05,p=0.01).The mean VAS score was 1.6±1.1 in group I which was significantly higher than that(0.4 ± 0.6)in group II(p<0.05,p=0.02).The improvement either in shoulder function or pain relieve in group II was significantly superior than group ?.Furthermore,there were more blood loss during surgery in group I than group II(P=0.01),but no significant differnces were observed on the basis of hospital stay(p=0.2)and duration of surgery(p=0.5).CONCLUSION:Both hook plate fixation and double Endobutton technique yield satisfactory clinical outcome in the treatment of ACJ dislocation.However in the present study,compared to hook plate fixation the double Endobutton technique have better shoulder fnction recovery and pain relieve.
