Objective: 1.To study the expression and significance of ER-alpha in papillary thyroid carcinoma.2.1.To investigate the effect of estrogen and estrogen receptor inhibitor(Fulvestrant)on the growth of ER-alpha positive papillary thyroid carcinoma cells.Methods: 1.We collected 60 cases of papillary thyroid carcinoma and 20 cases of benign thyroid tumors as control groupat at the first affiliated hospital of university of South China between 2015 and 2016;To test the expression level of ER-alpha by immunohistochemistry method in these tissues.Then analysis the correlation of clinical pathology with the patient gender,age,menstrual status,tumor size,number of lymph node cancer made,the capsular invasion and so on.2.Take fresh surgical specimens,confirmed thyroid papillary carcinoma,confirmed by immunohistochemical case for ER-alpha positive.We use the primary cell culture to papillary thyroid cancer cells and extend the stable cells after 3-4 times experiments,each type was divided into eight groups,respectively,the common culture medium,containing 10-11 mol/L,10-10 mol/L,10-9 mol/L contains beta estradiol medium and ordinary medium + fulvestrant group of 15 ng/ml,containing 10-11 mol/L beta estradiol + 15 ng/ml fulvestrant,10-10 mol/L beta estradiol + 15 ng/ml fulvestrant,10-9 mol/L beta estradiol + 15 ng/ml fulvestrant medium culture cell,application of CCK 8 method to detect cell proliferation and activity,the results using SPSS18.0 statistical analysis.Results: 1.ER-alpha in papillary thyroid carcinoma and normal or benign tissues expressed showed in the cytoplasm and cell membrane,but not the nucleus.ER-alpha in papillary thyroid carcinoma tissues(81.7%,49/ 60)and control group(20%,4/20)(P< 0.005)have significant differences;ER-alpha intensity expressed in thyroid papillary carcinoma in the case of(-20%,+ 23.3%,+ +26.7%;+ + + 30%).The expression of ER-alpha intensity level has no significant correlation(P > 0.05)with the sex of the patients(P = 0.519),age(P = 0.590),whether the menopause(P = 0.581),tumor diameter(P = 0.635),and whether the merger nodular goiter(P = 0.439),whether the combination of hashimoto's thyroiditis(P = 0.728);The expression level of ER-alpha and the number of patients with lymph node metastasis(P=0.046),envelope,vascular invasion(P=0.043),the number of lesions(P=0.039)is associated.The expression intensity increased of ER-alpha positive,the number of lymph node metastasis,capsule,vascular invasion,the number of lesions increased.2.The synchronous appearance of Hashimoto's thyroiditis and papillary thyroid carcinoma in 6 of 60 patients and ER-alpha express positive is 5 cases with tumor tissue.In control group,5 cases of patients with hashimoto's thyroiditis pathological section are shown positive expression of ER-alpha in the acinar tissue around the inflammation.3.The different concentration single-agent 17 beta estradiol group and 17 beta estradiol + 15 ng/ml fulvestrant group department applies to the ER positive thyroid papillary carcinoma after 120 hours,different concentrations of thyroid papillary carcinoma OD value of the normal control group were increased(P < 0.05);And increased with the increase of drug concentration;Comparing two groups of absorbance values were statistically significant(P < 0.05).But 17 beta estradiol + 15 ng/ml fluoride group department is corresponding to the same concentration compared single-agent 17 beta estradiol group,small OD value increase.Single-agent 15 ng/ml fulvestrant group department and ordinary medium group OD value no significant statistical difference(P > 0.05).Conclusion: 1.The expression of ER-alpha in papillary thyroid carcinoma was significantly different from that of benign tumor or normal tissue,and its expression intensity was related to the stage,progression and prognosis of patients with papillary thyroid carcinoma 2.ER-alpha may mediate and bridge between Hashimoto's thyroiditis and thyroid papillary carcinoma' development and occurrence.Part 2: The effect of Fulvestrant on the growth of ER-alpha positive thyroid papillary carcinoma cells 3.Estrogen can promote ER-alpha papillary thyroid cancer cell growth,and in a concentration dependent manner? 4.Fulvestrant suppress the estrogen stimulation of ER-alpha positive papillary thyroid cancer cell growth.
