Influence Of Sini Dispersed And Sijunzi Decoction On Gastrointestinal Function Recovery For The Liver And Spleen Deficiency Patients After Liver Transplantation

ObjectiveStudy on the clinical role of Sini Dispersed and Sijunzi Decoction in promoting the recovery of gastrointestinal function and reducing the incidence of abdominal distention after liver transplantation.compare the differences of the early recovery of gastrointestinal function after liver transplantation between the used group and the unused group.Provide clinical basis of traditional Chinese medicine,in promoting the recovery of gastrointestinal function after liver transplantation and reducing the complications.MethodSelect the clical data of patients in Hepatobiliary Surger,Fuzhou General Hospital who underwent original orthotopic liver transplantation patients,from July 2015 to January 2017.Using prospective cohort study,twenty-two patients were selected as control group,eighteen patients were selected as treatment group.1.Patients of the control group were fasting,forced water,persistent gastrointestinal decompression,keep the water,electrolyte,acid-base'balance.Antibiotics were given to against infection,immunosuppressive agents were given to prevent form anti-rejection,At the same time,the patients were given the intestinal nutrition by the stomach jejunum fistula.2.The patients of the treatment group,not only had the routine treatment of the control group,but also were given the Sini Dispersed Sijunzi Decoction.The first day,twice a day,each time 50ml,the second and the third day,twice a day,each time 100ml,through the stomach jejunum fistula injection.3.Both groups of the TCM syndrome score and the gastrointestinal function score were recorded separately.The bowel sounds recovery time(T1),the first exhaust time(T2),the first defecation time(T3).Alanine aminotransferase,total bilirubin,plasma ammonia,postoperative T tube bile drainage of the first day after operation,the third day,the fifth day,the seventh day and the ninth day were recorded.Result1.By T tset,we can know that the recovery time of bowel sounds(T1),the first exhaust time(T2)and the first defecation time(T3),the data of the treatment group was significantly less than the control group,they have the statistical differences(P<0.05).2.At the TCM syndrome score and the gastrointestinal function score,the treatment group had statistical differences from the control group by repeated measurement of variance analysis and Ridit analysis(P<0.05),the treatment group was less than the control group.3.Through the two-factor repeated measurement of variance analysis,the clinical data of both groups had the significant difference with the time goes by(P<0.05),the plasma ammonia between the treatment group and the control group had a statistical difference(P<0.05),the degree of declining of the treatment group was better than the control group.Conclusion1.Sini Dispersed and SiJunzi Decoction can significantly speed up the gastrointestinal function's recovery,ease bloating's happened,promote the organs recovery after the surgery,protect the transplant liver's function recovery.2.Sini Dispersed and SiJunzi Decoction can increase the ease of the liver and spleen deficiency,restore liver catharsis and spleen of the transport.3.Sini Dispersed and Sijunzi Decoction can accelerate the decline the plasma ammonia after surgery,decline the incidence rate of the Hepatic encephalopathy.
