Objective: To explore application and short-term clinical effect of the Short Monolithic Femoral Stem in primary total hip replacement. Methods: Patients were collected from October 2015 to December 2016 in the first affiliated hospital of Suzhou university with the Short Monolithic Femoral Stems implanted in primary total hip replacement. There were 6 males and 9 females, with an average age of 60 years.Preoperative diagnosis included avascular necrosis of the femoral head in 2 cases, femoral neck fractures in 5 cases, developmental dysplasia of the hip in 4 cases, osteoarthritis in 4 cases. The adopted femoral component was the Short Monolithic Femoral Stem, the junction of the head mortar interface were used: O xinium-high cross-linked polyethylene-R3 acetabular cup combination.Joint head using standard(28mm)8 hips,non-standard head(32mm)7 hips.12 cases were treated with direct anterior approach(DAA),and 3 patients underwent conventional posterolateral approach.Clinical and radiographic evaluations were performed on every follow-up.Results: The mean follow-up was 8 months(range 3 to 14 months). The mean preoperative Harris hip score was 51.94 ± 20.09,which were improved to 91.14±4.27(P<0.01)at the time of 1 months to 3 months. At the time of follow-up,Harris hip score was 94.54±4.57(P<0.01).At the time of follow-up,there were no cases of thigh pain. The X-ray films showed that no socket demonstrated migration, loosening and sinking.During prosthesis placement, postoperative infection, deep vein thrombosis in the lower limb, joint dislocation, fractures surrounding the prosthesis and prosthesis loosening was not observed.1 case of lateral femoral nerve injury was observed.1 case of postoperative imaging Gruen zone 7 appeared bone absorption after 1 year, but hip joint activities of the patient was good,no pain and so on.Conclusions: The short-term clinical effect of applying the Short Monolithic Femoral stem in primary total hip replacement is satisfactory. Not only did it reserve the volume of bone mass, but also effectively improved the function of hip joint. We can obtain better initial postoperative stability.
