Objective: To investigate and analyze the effect of the retention of the joint capsule on the postoperative clinical efficacy of hip arthroplasty in the treatment of elderly osteoarthritis. Methods: A retrospective analysis of 120 elderly patients with hip osteoarthritis who were admitted to hospital in July October 2010 ~2017 was conducted, including 60 patients in the study group and 60 patients in the control group. In the study group, the joint capsule was retained and repaired during hip replacement, while the control group cut off the joint capsule in the hip replacement. The indexes of operation in two groups were compared and observed, including operative time, intraoperative blood loss, postoperative 24 hour drainage volume, postoperative infection rate, dislocation in 6months after operation, hip joint function Harris score at 1 months and 12 months after operation. Results: In the 1 months after operation, the Harris score of the hip joint function in the control group was(86.9 + 9.06),while that in the study group was(89.97 + 7.10),which was better than that in the control group(P<0.05).However,12 months after the operation, two groups of hip functional Harris scores were equal(P>0.05).The good rate of hip joint function in the study group 1 months after operation was(88.33%),the control group was(70%),and the difference was statistically significant after statistical analysis,?2=6.114,P<0.05.But at the end of the follow-up,the study group(96.67%)and the control group(90%)had no significant difference after statistical analysis,?2=2.143,P>0.05.The operative time in the study group was(94.8 + 13.7)min,intraoperative blood loss was(268.9 + 25.6)ml,postoperative drainage volume was(157.3 + 19.8)ml,1 cases of postoperative dislocation,no postoperative infection. The operative time in the control group was(94.7 + 12.4)min,the intraoperative blood loss was(265.1 + 22.3)ml,the postoperative drainage volume was(154.3 + 10.2)ml,2 cases of postoperative dislocation,no postoperative infection. After statistical analysis, there was no statistically difference in operative time, intraoperative blood loss,postoperative24 hour drainage volume, postoperative infection rate and dislocation rate within 6months after operation in the two groups(P>0.05).Conclusion: Hip arthroplasty in the treatment of elderly patients with hip osteoarthritis with the retention and repair of articular capsule can promote early functional rehabilitation of joints, effectively avoid postoperative infection, joint dislocation and other complications, and significantly improve the clinical efficacy.
