Objective:To study and analyze the effect of retention and resection of the joint capsule on the clinical outcome of patients undergoing DAA total hip arthroplasty.Methods:Seventy-five patients with DAA total hip arthroplasty admitted to our hospital from June 2016 to September 2017 were randomly selected.The study group was divided into control group and observation group according to different research methods.For example,DAA was performed with total hip arthroplasty for joint capsule resection;38 patients in the observation group were treated with DAA total hip arthroplasty for joint capsule retention(protected after T-cut switch capsule and suture repair after reduction of joint)Joint capsule).The operation time of the two groups,the drainage volume on the first day after surgery,the blood transfusion volume,the Harris score and the VAS pain score were observed.Results:(1)After operation,the operation time of the observation group was significantly lower than that of the control group,which was statistically significant(P<0.05).However,there was no significant difference in the drainage volume and blood transfusion between the two groups on the first day after surgery.Statistically significant,P>0.05.(2)In the first 3 months after surgery,Harris score and VAS pain score of the two groups were statistically significant,P < 0.05.After surgery,Harris scores of patients in the observation group were significantly higher at 14 days than those in the control group,and VAS pain scores were significantly lower than those in the control group.The comparison between the two groups was statistically significant(P < 0.05).However,3,6,9and 18 months after surgery,there was no significant difference in Harris score and VAS pain score between the two groups(P >0.05).(3)Postoperative complications occurred in a total of 1 case(2.63%)in the observation group,including 1 case of incision infection.In the control group,postoperative complications occurred in 3 cases,accounting for 8.11%,including 1 case of joint dislocation,1 case of nerve paralysis,and 1 case of incision infection.There was no significant difference between the two groups,P > 0.05.Conclusion:Joint capsule retention in DAA total hip arthroplasty,retaining articular capsule to reduce patient flow,blood transfusion and postoperative complications,but the operation time is longer,intraoperative risk increases,higher requirements on the performer;There was no significant difference in Harris score and VAS pain score between the two groups at 3 months,6 months,9 months and 18 months.
