OBJECTIVE:To study the clinical characteristics and risk factors of gender,age,fracture types,concomitant diseases,inpatient cost and combined with other site fractures in elderly patients with hip fracture which would help to the understanding of hip fracture in the elderly.METHODS:The clinical data of 1385 elderly patients with hip fractures who were treated in the Zhongda Hospital from January 2005 to December 2014 were collected and analyzed.To analyse gender,age,fracture types,concomitant diseases,inpatient cost and combined with other site fractures.According to whether the patients combined with other site fractures were divided into combined fractures group and nonunion combined fractures group,the gender,age,fracture types and concomitant diseases of the two groups were compared,and the factors of P<0.05 were analyzed by logistic regression analysis.RESULTS:? gender and age distribution:A total of 1385 cases of hip fractures were collected,including 460 cases of male(33.21%),female 925 cases(66.79%),male to female ratio of 1:2.01.The age range was 60?109 years,the average age was(80.45±9.13)years,and the high incidence age was 80?90 years(604 cases,accounting for 43.61%of the total).?fracture types:Femoral intertrochanteric fractures 596 cases(43.03%);femoral neck fracture 789 cases(56.97%),the ratio was 1:1.32.Most of them were female.The average age of patients with femoral neck fractures was(78.86±9.233)years and the mean age of patients with intertrochanteric fractures(82.57±8.546)was statistically different(P<0.05).The proportion of the intertrochanteric fractures increased gradually with age.?concomitant diseases:1003 patients with concomitant diseases(72.42%),without the incidence of 382 cases(27.58%).There were 321 patients(23.18%)with one disease,269 patients(19.42%)with the two diseases,and 413(29.82%)with three or more concomitant diseases.Common diseases include cardiovascular disease(47.22%),endocrine disease(22.09%),neurological diseases(21.95%),and respiratory disease(9.53%).From 2005 to 2014,the average number of patients with hip fractures in the elderly showed an upward trend.? The cost of hospitalization:the average cost of hospitalization(33864.54 ±17141.13)yuan,intertrochanteric fracture(33789.99 ± 17397.03)yuan and femoral neck fracture(33920.86±16956.12)yuan,there was no significant difference between the two groups(P>0.05).From 2005 to 2014,the average hospital cost of elderly patients with hip fractures had no significant upward trend.the total hospital costs were on the rise ?combined with other site fractures:1385 cases of elderly patients with hip fractures,combined with other parts of the fracture of 112 patients(8.1%).There were 20 males and 92 females with a ratio of 1:4.6.The age distribution was 61?99 years old,with an average of(82.00 ± 8.80)years.There were 79 femoral neck fractures and 33 femoral intertrochanteric fractures.The high incidence age Combined with fractures in the elderly combined with fractures was 80?89 years.With the age increased,the account of the patients combined with fracture also raise.Above 90 years elderly hip fracture patients with the rate of 11.27%.A total of 130 parts of the fracture,commonly combined fracture site:ulnar radius 36.15%,humeral 17.69%,thoracolumbar vertebrae 16.15%,femur 10.00%.There were significant differences in gender,fracture type,cardiovascular disease,endocrine system diseases and neurological diseases between the two groups(P<0.05).The age,respiratory disease,digestive system diseases,urinary system diseases were not statistically significant(P>0.05).Logistic regression analysis showed that gender,fracture type,cardiovascular disease and endocrine system disease(diabetes)were the major risk factors for hip fractures in the elderly(P<0.05).CONCLUSION:1.The elderly hip fracture more common in women,high incidence of fracture age 80?90 years,femoral neck fracture was more common than femoral intertrochanteric fractures,but the proportion of the intertrochanteric fractures increased gradually with age.2.Most of elderly patients with hip fracture had concomitant diseases.Common concomitant diseases include:cardiovascular disease,endocrine system diseases,neurological diseases,respiratory diseases.3.The number of elderly patients with hip fractures,the number of concomitant diseases,the total inpatient cost were on the rise.4.The common site of the fracture is:ulnar radius,humerus,thoracolumbar vertebrae,femur.Gender,type of fracture,cardiovascular disease,endocrine system disease(diabetes)is an independent risk factor for hip fractures in the elderly combined with other site fractures.
