Migraine is an idiopathic headache disorder that has been an important healthcare and social problem for its great influence on the quality of life,accompanied by severe headaches,nausea and light sensitivity.Placebo effects are a powerful illustration of a learning phenomenon that can produce treatment outcome via verbally-induced expectations and cued and contextual conditioning,which are mediated by a direct engagement of the brain in processing cognitive inputs.Under the background of pain to the placebo effect caused by the neural mechanism of research has shown that hope for pain relief can activate pain regulation system,therefore,the placebo effect in chronic pain will reach even more than the real effect of treatment.The placebo effect is intensively studied in healthy volunteers by using functional MRI However,there is little knowledge about the predictive role of the brain in chronic pain patients.This study was treated with placebo for induced analgesia,all patients participated in an8-week sham acupuncture treatment to induce analgesia.MWo A patients were subdivided(50% change in migraine attack frequency from baseline)into recovering and persisting patients.based on 3D T1 structural image and functional magnetic resonance image,using voxel-based morphometry method and functional connectivity analysis method,we assessed the difference of the local gray between MOr and MOp,we explored the index that could predict placebo analgesia.At the global level,based on DTI and graph theory approaches,using the fiber bundle tracking technology,as well as the complex network theory to explore the potential abnormal connections within the brain structural network of MOr and MOp.The main results of this paper are as follows:1.compared with healthy and MWo Ap,baseline brain gray matter(GM)density of the medial prefrontal cortex(m PFC)and its related local functional connectivity exhibited lower values in the MWo Ar.2.Selected hippocampal brain regions as a seed region,we conducted the functional connectivity analysis.The results showed that,the intensity of the m PFC-amygdala connection exhibited significant between-group differences between the MWo Ap and MWo Ar,and the extent of this connection was coupled with GM changes in the m PFC and was directly related to the therapeutic effect exhibited in the MWo Ar.3.we analysis the structural network properties.The results showed that,compared with healthy and MWo Ar,the network topology of the structural network significantly changed in the MWo Ap,and the brain connetome is closely associated with the patient's placebo treatment.
