Purpose: Parkinson's disease is a common neurodegenerative disorder.Its etiology and pathogenesis are very complicated.Early detection and early treatment can improve the quality of life of patients,and patients with poor drug treatment can choose surgical treatment.Voxel-based morphometry(VBM)is a relatively new brain structure image analysis technology with comprehensive,objective and unbiased advantages.The brain structure covariation network is constructed on the basis of VBM.The structure of the brain network,its emergence opened up a new way for brain structure analysis.In this study,the VBM technique and the brain network constructed on this basis were used to investigate the correlation between brain structural changes and surgical outcomes in patients with Parkinson's disease after nuclear destruction,so that the results can help clinicians quantify lesions before surgery.Target volume,better surgical results.Methods: Twenty-one patients with Parkinson's disease who underwent surgery on the functional neurology department of Shanghai Oriental Hospital from April 2017 to June 2018 in the Department of Functional Neurology of the Shanghai Municipal Hospital for the treatment of all patients with Parkinson's disease.Pre-and post-clinical data collection,including United Parkinson Diseases Rating Scale(UPDRS),Hoehn-Yahn classification(HY classification),levodopa equivalent dose LED calculation,and simultaneous surgery Whole brain MR scans were performed 10 days before and after surgery to obtain 3D high resolution T1-weighted structural images.The data analysis is based on matlab13 b platform,CAT12 software package for structural image preprocessing,spm12 for statistics,Mricron software for results presentation,and the data obtained into SPSS19 for correlation analysis;BCT software package for gray matter structure covariation network index calculation,Brain NetViewer Used for visualization of charts.Results: After the nucleus destruction,the symptoms of Parkinson's disease in the opening and closing period and the dose of levodopa were significantly improved,and there was a significant positive correlation.The tremor was the core motor symptom,and the improvement degree was positively correlated with the Vim volume of the damaged target.The improvement of post-medicinal amount was also positively correlated with the volume of Vim;the small world attribute of brain network of patients after destruction was changed,and the overall efficiency was improved.Conclusion: Surgery improves the clinical symptoms of Parkinson's disease,and patients with poor drug treatment can choose Parkinson's surgery.Under the assumption that the target location is accurate,the size of the surgical target is closely related to the postoperative efficacy,and the target volume should be reasonably quantified before surgery.Surgery changes the brain network of patients,and brain network changes should be taken into account when developing surgical plans.
