Objectives 1.Using ventricular strain index by ultrasound speckle tracking imaging to study the changes of left ventricular function in patients before and after CABG operation.2.Using ultrasound speckle tracking imaging to study the correlation between left atrial strain index and postoperative atrial fibrillation.Methods 53 cases of CABG patients in cardiac surgery department of Hebei University Affiliated Hospital of from Jan.1st 2016 to Oct.30 th 2016 were selected,and 50 normal persons were selected as control group,and cases were selected as control group.1.Using transthoracic echocardiography to measure normal and CABG patients in the preoperative and postoperative 4-7d,1 months septal thickness(IVST),left ventricular diameter,left ventricular ejection fraction(LVEF),and using STI technology to measue normal people and patients with CABG before and after operation.4-7d,after 1 months of monitoring the changes of left ventricular function index.2.Using transthoracic echocardiography to measure normal and CABG patients in the preoperative determination of left atrial diameter(LAD),early diastolic transmitral flow velocity(E),late diastolic transmitral flow velocity(A),E/A;using STI technology to measure normal people and patients with CABG preoperative measurement of left ventricular long axis left atrial systolic strain(Ss),left atrial systolic long axis left atrial strain(Sa)and the whole atrial segmental strain rate(SRs),continuous ECG monitoring was performed in patients after operation for at least 72 hours.Result(1)The absolute value of the application of STI technology were compared between the two groups of two chamber view,four chamber view and three chamber view overall strain value,the overall strain of left ventricle in patients with CABG value lower than the control group,with the prolonged postoperative graft blood supply time,each segment of the overall strain value was increased compared with the preoperative,the difference was statistically significant(P<0.05).(2)Compared with the control group,the indexes of postoperative atrial fibrillation,had higher E,lower A,higher E/A,the difference was not statistically significant(P>0.05),LAD,Ss,Sa,SRs were lower,the difference was statistically significant(P<0.05).(3)Compared with the control group,there was no significant difference in the indexes of postoperative non atrial fibrillation group(P>0.05).(4)Compared with non atrial fibrillation group,Ss,Sa and SRs were lower in patients than postoperative atrial fibrillation.The difference was statistically significant(P < 0.05).Conclusions(1)STI technique can evaluate left ventricular systolic and diastolic function in patients with CABG non-invasively and quantitatively,which provides a powerful new tool for the evaluation of left ventricular function before and after CABG.(2)The value of left atrial strain measured by STI technique before operation is significantly correlated with the incidence of new onset atrial fibrillation after CABG,when LAD and BMI are much higher,Ss,Sa,and SRs are much lower,and atrial fibrillation is easy to happen,these results suggest that LAD,BMI,Ss,Sa and SRs are risk factors for the occurrence of atrial fibrillation after CABG.
