Objective: The study aimed to analyze the prevalence,clinical features and correlates of thicker carotid intima-media and carotid plaque among the type 2 diabetes mellitus(T2DM)and to prevent or treat patients through finding high risk group early,which will provide theoretical basis for the clinical application in reducing the occurrence and fatality rate of type 2 diabetic cardiovascular disease.Methods:A retrospective review was conducted in Metabolic Disease Hospital of Tianjin Medical University between January 1,2008 to March 1,2013 extracting relevant information from medical records of T2 DM inpatients(2525).Patients with neck ultrasonic testing(1167)were enrolled in this study,then we analyzed the prevalence and clinical features of thicker carotid intima-media and carotid plaque among T2 DM inpatients.A case-control study was used,which the subjects with thicker carotid intima-media(604)and carotid plaque(162)were as case groups respectively and the subjects with normal carotid intima-media thickness(401)as control group.To analyze correlates of thicker carotid intima-media(604)and carotid plaque,data were analyzed using unconditional logistic regression and OR value and 95%CI were calculated.Results: 1.604 had thicker carotid intima-media(51.8%)and 162 had carotid plaque(13.9%)among the inpatients with neck ultrasonic testing;thicker carotid intima-media and carotid plaque increased with the advanced age and the longer duration of diabetes among the inpatients of type 2 diabetes mellitus.2.Among the inpatients of type 2 diabetes mellitus,compared to normal carotid intima-media thick group,the thicker carotid intima-media and carotid plaque groups tend to be older,working staffs,retirees,have longer duration of diabetes,have coronary heart disease,hypertension and diabetic retinopathy and higher SBP,BUN,CRE and FIB levels;the carotid plaque group tends to be have higher UPRO level and smokers;the thicker carotid intima-media group tends to receive insulin treatment and oral hypoglycemic drug treatment,and higher UMA levels.3.In the univariate logistic regression analysis,the results showed that men,advanced age,retirement,smoking,obesity,longer duration of diabetes,insulin treatment,oral hypoglycemic drug treatment,diabetic retinopathy,diabetic nephropathy,hypertension,coronary heart disease,FIB >4.0g/L were associated with higher odds of thicker carotid intima-media among type 2 diabetic inpatients;men,advanced age,retirement,smoking,longer duration of diabetes,diabetic retinopathy,coronary heart disease,hypertension,BUN ?8.3 mmol/L? FIB >4.0g/L? CRP ?8 mg/L were associated with higher odds of carotid plaque among type 2 diabetic inpatients.Significant factors in univariate logistic regression analysis were included in the multivariate unconditional logistic regression,and results showed that advanced age,men,obesity,hypertension,and coronary heart disease were associated with higher odds of thicker carotid intima-media among type 2 diabetic inpatients;advanced age,smoking,FIB >4.0g/L,coronary heart disease were associated with higher odds of carotid plaque among type 2 diabetic inpatients.Conclusions:1.The prevalence of thicker carotid intima-media and carotid plaque among type 2 diabetic inpatients was high;Both the prevalence of thicker carotid intima-media and carotid plaque increased with advanced age and longer duration of diabetes.2.Advanced age,men,obesity,hypertension,and coronary heart disease were associated with high odds of thicker carotid intima-media;Advanced age,smoking,higher FIB levels,coronary heart disease were associated with high odds of carotid plaque formation among type 2 diabetic inpatients.
