Background&Objective:With the increasing incidence of type 2 diabetes mellitus(T2DM)and its complications,T2DM,especially diabetic complications,has seriously affected the quality of life and health and safety of diabetic patients.Among the complications of diabetes mellitus,the highest mortality rate is amputation caused by cardiovascular and cerebrovascular diseases and lower extremity vascular lesions,and the main pathological features of cardiovascular and cerebrovascular diseases and lower extremity vascular lesions are atherosclerosis.Early detection showed that the treatment of atherosclerosis in diabetic patients can effectively reduce the disability rate and mortality.The thickening of carotid intima-media thickness(Carotid artery intima-media thickness,CIMT)or the formation of carotid plaques is a reliable index to evaluate atherosclerosis and is a high risk factor for stroke,coronary heart disease and other cardiovascular and cerebrovascular diseases.In the past,it was thought that atherosclerosis was mainly caused by disorder of glucose and lipid metabolism and hypertension,but recently it was found that hyperuricemia also played an important role in the occurrence and development of atherosclerosis.Some studies suggest that there is a significant correlation between hyperuricemia(HUA)and type 2 diabetes mellitus complicated with vascular complications.With the change of diet structure,the occurrence of hyperuricemia also showed an obvious upward trend.Therefore,the purpose of this study was to compare blood glucose,glycosylated hemoglobin,blood lipid,carotid intima-media thickness and carotid plaque formation in patients with type 2 diabetes mellitus,and to explore the relationship between serum uric acid and atherosclerosis in patients with type 2 diabetes mellitus,so as to provide some reference for early detection,prevention and treatment of carotid atherosclerosis in patients with type 2 diabetes mellitus.MethodsFrom December 2015 to December 2017,300 patients with type 2 diabetes who were admitted to Henan Electric Power Hospital were selected as subjects,and general information such as gender,age,height and weight of all patients were collected and necks were collected.Ultrasound findings of arterial ultrasound and fasting blood glucose(FPG),glycosylated hemoglobin(HbAlc),total cholesterol(TC),low density lipoprotein(LDL-C),triglyceride(TG),high density lipoprotein(HDL-C),The results of biochemical tests such as serum uric acid(SUA)were divided into group A(man ? 416 ?mol/L,female?357 ?mol/L)and group B(men>416 ?mol/L,female>357 ?mol/L)according to blood uric acid level.The differences of general data,biochemical findings,carotid intima-media thickness and carotid plaque formation were compared between the two groups.Spearman correlation analysis and logistic regression analysis were used to investigate the relationship between serum uric acid and carotid intima-media thickness and carotid plaque.The relationship of block formation.Results:(1)There were 195 cases in group A and 105 cases in group B.The male proportion in group A was significantly lower than that in group B,the course of diabetes mellitus was significantly longer than that in group B,systolic blood pressure,diastolic blood pressure and TC,TG,LDL-C,FPG,HbAlc were significantly lower than those in group B,while the level of HDL-C in group A was significantly higher than that in group B(P<0.05).There was no significant difference in age and BMI between the two groups.(2)In this study,103 cases of carotid intima-media thickening were detected.The detection rate of carotid intima-media thickening was 34.33%(103/300).The intima-media thickness of carotid artery in group A was(0.81 ± 0.46)mm,and the intima-media thickness of carotid artery in group B was(1.15±0.67)mm;The thickness of carotid intima-media in group A was significantly lower than that in group B(P<0.05).In group A,58 cases of carotid intima-media thickening were detected,and the detection rate of carotid intima-media thickening was 29.73%(58/195).In group B,45 cases of carotid intima-media thickening were detected,and the detection rate of carotid intima-media thickening was 42.86%(45/105).The detection rate of carotid intima-media thickening in group A was significantly lower than that in group B(P<0.05).(3)In this study,47 cases of carotid plaque formation were detected,and thedetection rate of carotid plaque formation was 15.67%(47/300).A group,24 cases of carotid plaque formation were detected,and the detection rate of carotid plaque formation was 12.31%(24/195).23 cases of carotid plaque formation were detected in group B,and the detection rate of carotid plaque formation in group B was 21.90%(23/105).The detection rate of carotid plaque formation in group B was significantly lower than that in group B(P<0.05).(4)Spearman correlation analysis showed that there was a significant positive correlation between serum uric acid level and carotid intima-media thickness.Logistic regression analysis was used to correct the effects of sex,course of diabetes,blood pressure and TC,TG,LDL-C,HDL-C,FPG,HbA1c.It was found that serum uric acid was an independent risk factor for carotid intima-media thickening and carotid plaque formation.ConclusionFor type 2 diabetes patients,serum uric acid level is related to the course of diabetes,blood glucose control,lipid metabolism disorder,hypertension and so on.The incidence of carotid intima-media thickening and carotid plaque formation is high in patients with type 2 diabetes mellitus.There is a significant positive correlation between serum uric acid level and carotid intima-media thickness,and it is an independent risk factor for carotid intima-media thickening and carotid plaque formation,which is of great significance for the prevention and treatment of vascular complications in diabetes mellitus.In order to reduce the occurrence of vascular complications and improve the prognosis of diabetic patients with elevated serum uric acid level,attention should be paid to the diagnosis and treatment.
