Construction Of Standardized Nursing Care System Of Pressure Ulcer For Inpatients

Objective1.This study aims to understand the status of assessment,prevention,treatment,management of pressure ulcers and provide reference for establishing inpatients' standardized pressure ulcer nursing care system,by searching and analyzing a large number of domestic and foreign literature and guidelines on pressure ulcers.2.Building a scientific,systematic and comprehensive inpatients' standardized pressure ulcer nursing care system according to the national conditions,which can provide the reference of assessment,prevention,treatment,management and other aspects on pressure ulcers for nursing practitioners and nursing managers.Methods1.Searching literature from PubMed,Cochrane library,EMbase,BMJ,EBSCO,Chinese Biomedical Literature Database(CBM),CNKI,Wan fang database,searching guidelines from National Guideline Clearinghouse(NGC),the time limit is from 2006 to 2015.Reading the abstract and the full text of the literature to choose the literature in accordance with the inclusive criteria,then evaluating and extracting information from those literature.After discussion and analysis,the first draft of inpatients' standardized pressure ulcer nursing care system is finished.2.Using the Delphi method to evaluate the importance and operability of the items.By analyzing and discussing to change,add and delete the items,inpatients' standardized pressure ulcer nursing care system was built.Results1.Based on the evidence-based nursing,the first draft of inpatients' standardized pressure ulcer nursing care system was built,including 4 first-level items,19second-level items and 79 third-level items.2.The effective rates of the two rounds of questionnaires were 93.75% and 100%respectively.The ratio of experts who proposed the revised opinions was 83.33% and16.67% respectively.The authority coefficient of the two rounds' questionnaires were0.91 and 0.92 respectively.The experts' coordination coefficients of the importance were 0.251 ~ 0.465(P<0.05),and the experts' coordination coefficients of the operability were 0.235 and 0.452(P <0.05).3.In the first round,about the importance score of the first-level items,the average was 4.43~4.93,the variation coefficient was 0.06~0.16;about the importance score of the second-level items,the average was 4.13~4.80,the variation coefficient was0.09 ~ 0.17;about the importance score of the third-level items,the average was3.50~4.83,the variation coefficient was 0.09~0.34;about the operability score of the third-level items,the average was 3.20 ~ 4.83,the variation coefficient was0.08~0.32.4.In the second round,about the importance score of the first-level items,the average was 4.60 ~ 5.00,the variation coefficient was 0.00 ~ 0.15;about the importance score of the second-level items,the average was 4.33~4.87,the variation coefficient was 0.07~0.16;about the importance score of the third-level items,the average was 4.27 ~ 4.97,the variation coefficient was 0.04 ~ 0.17;about the operability score of the third-level items,the average was 4.07~4.97,the variation coefficient was 0.04~0.23.5.After two rounds of expert correspondence,inpatients' standardized pressure ulcer nursing care system was built,which was including 4 first-level items,23second-level items and 85 third-level items.ConclusionEvidence-based nursing,and the Delphi method was used to construct inpatients' standardized pressure ulcer nursing care system,which was including 4 first-level items,23 second-level items,85 third-level items and consist of risk assessment,prevention,treatment and management.The system can provide scientific,standardized nursing care about pressure ulcers for nursing practitioners and nursing managers.
