The Effect Of Drug Combined With Mindfulnessbased Cognitive Therapy On Psychosocial Functioning Of Major Depressive Disorder

Objective: Major depressive disorder is a kind of mental illness which is characterized by high recurrence and has a serious damage on social function and life quality of patients.This article discusses the effect of MBCT combined with drug on improving the psychosocial function of patients with depression and provides a new way for the treatment and rehabilitation of patients with depression.Methods: 90 patients with depressive episode or recurrent depression according with the ICD-10 were enrolled as subjects.They were out-patients in Tianjin Anding Hospital from December in 2014 to December in 2015.All the patients were divided into MBCT group and escitalopram group at random.The MBCT group received drug therapy additional with MBCT.In escitalopram group,the patients only received drug therapy.Before and after treatment,HAMD-17,BDI-?,CGI,SF-36,SPSS,CSQ,SAQ,SES,SSPM and FFMQ were administered to all subjects.Results: 42 patients in MBCT group and 41 patients in escitalopram group finished the study,7 patients dropped off.Before the treatment,no significant differences between two groups in general condition(P>0.05).(1)After the treatment,the scores of HAMD in MBCT group were lower than that in escitalopram group,the difference was statistically significant(t=-2.645,P=0.009).(2)After the treatment,the scores of BDI-? in MBCT group were lower than that in escitalopram group,the difference was statistically significant(t=-2.949,P=0.004).(3)After the treatment,the scores of CGI-S of patients in MBCT group were lower than that in escitalopram group,the difference was statistically significant(t=-4.661,P=0.000);after treatment,the scores of CGI-I of patients in MBCT group were lower than that in escitalopram group,the difference was statistically significant(t=-2.397,P=0.019).(4)After the treatment,the scores of Physical Functioning,Role-Physical,Bodily Pain,General Health,Vitality,Social Functioning,Mental Health and the total score of MBCT group were higher than that in escitalopram group after treatment,the differences were significant(t=3.418,P=0.001;t=2.065,P=0.042;t=3.384,P=0.001;t=-3.249,P=0.002;t=3.249,P=0.002;t=6.583,P=0.000;t=4.354,P=0.000;t=4.775,P=0.000;t=8.739,P=0.000).(5)After the treatment,the scores of SDSS in MBCT group werelower than that in escitalopram group,the difference was statistically significant(t=-4.181,P=0.000).(6)After the treatment,the scores of solving problems and recourse of CSQ in MBCT group were higher than in escitalopram group,the differences were statistically significant(t=4.114,P=0.000;t=2.780,P=0.007);the scores of self-accusation and wince of CSQ in MBCT group were lower than in escitalopram group,the differences were statistically significant(t=-4.413,P=0.000;t=-2.002,P=0.031).(7)After the treatment,the scores of self-acceptance and the self-evaluation and the total score of SAQ of patients in MBCT group were higher than that in escitalopram group,the differences were statistically significant(t=2.759,P=0.007;t=2.011,P=0.048;t=-2.860,P=0.005).(8)After the treatment,the scores of SES of patients in MBCT group were higher than that in escitalopram group,the difference was statistically significant(t=5.588,P=0.000).(9)After the treatment,the scores of SSPM of patients in MBCT group were lower than in escitalopram group,the difference was statistically significant(t=-5.153,P=0.000).(10)After the treatment,the score of every factor and the total score of FFMQ of patients in MBCT group were higher than in escitalopram group,the differences were statistically significant(t=3.957,P=0.000;t=2.229,P=0.029;t=4.359,P=0.000;t=4.275,P=0.000;t=3.678,P=0.000;t=4.082,P=0.000).Conclusions: With the original drug,(1)clinical symptoms of patients with major depressive disorder were more significantly improved by MBCT than only by the original drug.(2)social functional and life quality of patients with depression were more significantly improved by MBCT than only by the original drug.(3)the ability of coping,the levels of the self-esteem and self-acceptance of patients with depression were more significantly improved by MBCT than only by the original drug,the level of the stigma of patients with depression was lower by MBCT than only by the original drug.(4)the level of the mindfulness of patients with depression was higher by MBCT than only by the original drug.(5)combining MBCT had positive influence on the psychosocial functioning of patients with major depressive disorder and makes patients with major depressive disorder more quickly returning to society than only by the original drug.
