Analysis On Orthodontic Treatment For Anterior Teeth Displacement Caused By Periodontal Disease

Objective: To investigate the changes of periodontal health index before and after periodontal basic treatment and orthodontic treatment,then discuss the application value of orthodontic treatment for anterior teeth displacement caused by periodontal disease.Methods: A total of 66 anterior teeth displacement patients caused by periodontal disease in the department in daqi of stomatology ng oilfield general hospital from July 2013 to May 2015 were elected.All of the patients were treated with periodontal basic treatment first,then were made orthodontic treatment after the periodontium stable.Observe the depth of the diagnosis,percentage of bleeding sites,clinical attachment loss,clinical crown length,root length and alveolar bone density changes in different areas before and after the periodontal basic treatment and combined orthodontic treatment.Observe the changes of the density of the upper and lower mandible before and after orthodontic treatment;Observe the orthodontic situation before and after orthodontic treatment,including the height of alveolar bone,periodontal pocket depth,bleeding index,anterior teeth overbite and anterior teeth cover changes.Measuring the relationship between jaws and teeth on lateral cephalometric before and after treatment.Then statistical analysis those datas.Results: Simple periodontal treatment can reduce the depth of the probing depth,the percentage of bleeding sites was significantly less than that before treatment,the loss of clinical attachment was significantly less than that before treatment(P<0.01).The length of the dental crown increased;the length of the root was not significantly changed compared with that before treatment.The alveolar bone density in the alveolar crest and apical area was significantly higher than that before treatment.After the combined orthodontic treatment,the depth of probing was further reduced,which was significantly different than that before treatment(P<0.01).The percentage of exploratory bleeding sites was further reduced,which was significantly different than that before treatment(P<0.01).There was no significant difference of clinical attachment loss compared with between orthodontic treatment(P>0.05).The length of the dental crown was further increased,and the difference was significant compared with before treatment(P<0.05).The length of the root was no different compared with before treatment(P>0.05).The alveolar bone density in the alveolar crest and the apical area were decreased significantly,and the difference was significantly compared with before treatment(P<0.05 or P<0.01).The upper and lower jaw density were significantly lower than that before treatment,and the difference was statistically significant compared with before treatment(P<0.01),and the lower jaw density decreased more significantly.All the anterior teeth were restored to normal relationship of coverage and covering,the alveolar bone height was significantly reduced,the periodontal pocket was shallow,the periodontal condition was improved,the bleeding index was significantly smaller,and the difference was statistically significant compared with before treatment(P<0.05).After orthodontic treatment,there are 6 patients with incisors scattered in the gap,accounting for 18.75%.The U1-APg was significantly smaller,AB-NPg,U1-L1 and NPg-FH were significantly larger,and the phenomenon of lips and mandibular retraction were significant improvement,MP-FH,OP-FH and L1-OP were statistically significant compared with those before treatment(P<0.05).Conclusion: Using periodontal basic treatment first,then made orthodontic treatment after the periodontium stable for patients on anterior teeth displacement caused by periodontal disease can significantly correct the anterior teeth displacement phenomenon,the clinical effect was well,it's well worth promoting.
