Study On The Characteristics And Prognosis Of Essential Hypertension In Young Adults

Objective:Retrospectively analysis the young adults with hypertension in the First Hospital of Shanxi Medical University,Department of Cardiology,and follow the hospital treatment compliance,to study the young adults with hypertension and the relationship between treatment and prognosis,the initial statement of comprehensive risk control(including drug therapy)on the importance of target organ protection,with a view to provide some help to the clinical work.Methods:A total of 314 patients were enrolled in the hospital for diagnosis and treatment of young hypertensive patients from January 2005 to December 2015.According to the level of blood pressure,the patients were divided into three groups: grade 1 hypertensive group,grade 2 hypertensive group and grade 3 hypertensive group.Through statistical analysis of data and related follow-up data of each group patients,study the characteristics of young hypertensive disease and the relationship between treatment and prognosis of the disease.Results:In this study,314 patients were enrolled.There were 213 males(67.8%)and 101females(32.2%)with an average age of 38.59±4.14(29-44)years.There were a total of 60(19.11%)in grade 1 hypertension,109(34.71%)in grade 2 and 145(46.18%)in grade 3hypertension.Male hypertensive patients are about 2 times of the female,and the grade 3patients with hypertension are the most(about 50%);nearly 70% of patients with family history of hypertension;in terms of lifestyle,all levels of youth hypertension patients have different degrees of BMI exceeded,smoking,drinking and other bad habits,and with the blood pressure level increase,smoking,drinking and the proportion of family history increased;young hypertensive patients have blood lipids,blood sugar,uric acid and other metabolic abnormalities,but no difference between different blood pressure levels;in terms of disease,about 20% of grade 3 patients with coronary heart disease and ischemic stroke,about 8% of grade 3 patients with heart failure,brain bleeding or chronic kidney disease.In addition,about 20% of patients were treated with monotherapy,and nearly 40%of patients were selected with CCB,followed by ACEI.About 70% of patients were treated with two or three drugs,most of which are based on beta-blockers.After intensive intervention,the majority of young hypertensive patients with blood pressure control in about 135/85 mm Hg,total cholesterol control at 4.5mmol/L,triglyceride less than1.8mmol/L,low density lipoprotein were less than 2.6mmol/L,high density lipoprotein were greater than 1.09mmol/L.In terms of lifestyle,BMI had no significant change(before and after intervention: 24.33±2.42 vs 23.88±1.89),smoking and alcohol consumption were reduced(before and after treatment: 67.52% vs 59.24%;63.69% vs 51.27%).In the field of drug treatment,the total number of subjects treated with monotherapy was significantly increased(18.15% vs 28.34%).Among them,monotherapy was the most significant(23.33% vs 76.67%),grade 2 and 3(28.44% vs 28.44%,8.28% vs 8.28%).There was no significant change in the total number of patients(67.20% vs 67.20%).The patients were either grade 2 or 3 hypertensive patients.In terms of target organ damage,left ventricular hypertrophy(25.80% vs 28.03%),carotid arteriosclerosis(8.60% vs 10.83%),coronary heart disease(15.61% vs 18.15%)increased,heart failure(5.10% vs 6.05%),ischemic stroke(14.01% vs 15.29%),hemorrhagic stroke(5.10% vs 6.05%),and chronic kidney disease(4.78% vs 5.41%)were slightly increased.Conclusion:(1)The incidence of hypertension in young adults mostly have a family history,most male,obese body and poor habits are more prone to disease,accompanied by metabolic abnormalities;(2)In the treatment of young hypertensive patients,attention should be paid to comprehensive risk factors control,in particular change the bad habits,choose the best individual treatment program,with a view to maximum protect his target organ.
