Objective:The subject by observing the pregnant women in different sugar serum osteocalcin and adiponectin level change,explore the possibility of gestational diabetes pathogenesis,is expected to provide new theoretical basis for future prevention and treatment of type 2 diabetes.Methods:According to 2013 ADA diagnostic criteria,based on 24 to 28 weeks pregnancy oral glucose tolerance test(OGTT)results: embrace 42 pregnant women diagnosed with gestational diabetes mellitus in gestational diabetes mellitus(GDM)group,28 pregnant women diagnosed as diabetic pregnancy in diabetic pregnancies(PGDM)group and 30 pregnant women into normal glucose tolerance(NGT)group.Detection of blood glucose,insulin,adiponectin(ADP),osteocalcin(OC),calculate the indexes of insulin resistance(HOMA IR)and islet beta cell function index(HOMA-beta),after statistics for correlation analysis.Results:1.Among three groups of age,blood lipid four indicators,and gestational age differences had no statistical significance(P>0.05),three groups of BMI before pregnancy and prenatal BMI increased differential difference had no statistical significance(P>0.05);2.PGDM group,GDM group,NGT group of FPG,blood glucose 2 hours after sugar load(2 hPG)and HbAlC showed a trend of decline,Comparative differences between groups were statistically significant(all P<0.01);3.PGDM group of FINS,HOMA-IR is significantly higher than the GDM group and NGT group(P<0.01),FINS,HOMA-IR in NGT group were lower than GDM group,respectively is similar between the two groups have statistical significance(P < 0.05);4.PGDM group,GDM group,NGT group of HOMA-β increasing trend,compare the differences between groups was statistically significant(all P<0.01);5.GDM,NGT group,the level of serum osteocalcin were higher than that of PGDM group(Respectively P<0.01,P<0.05),GDM group serum osteocalcin level higher than the NGT group,statistical differences between each groups(P <0.01);6.The serum levels of adiponectin in NGT group,GDM group,PGDM group is reduced,compare the differences between groups were statistically significant(all P<0.01);7.Pearson correlation analysis showed the GDM and NGT group serum osteocalcin level and FINS,HOMA-IR significant positive correlation,the correlation coefficient of 0.709,0.696,respectively(P<0.01);PGDM group serum osteocalcin level and FINS,HOMA-IR negatively correlated,correlation coefficient is 0.707,and 0.784(P<0.01);there was a significant negative correlation between ADP and HOMA-IR in the three groups,and the correlation coefficient is-0.473(P<0.01);Three groups of adiponectin and osteocalcin have no obvious correlation.Conclusion:1.PGDM group,the GDM group and group NGT FINS,HOMA-IR is reduced,in turn prompt pregnancy women with the deterioration of the sugar metabolic abnormalities,insulin resistance is more obvious;2.The pregnancy women under different sugar metabolic state serum adiponectin and negatively correlated with insulin resistance,NGT group,GDM group,PGDM adiponectin level is reduced,in turn prompt low serum adiponectin may contribute to insulin resistance,leading to sugar metabolism;3.GDM group,NGT group,the level of serum osteocalcin were higher than that of PGDM group,the GDM and NGT group serum osteocalcin and HOMA-IR significant positive correlation,PGDM group serum osteocalcin and HOMA-IR negatively correlated,speculated that elevated levels of serum OC may be a compensatory of early insulin resistance,with increase in resistance to insulin and blood sugar rising decompensation.
