Objective: GDM is a pregnancy before normal glucose metabolism, Pregnancy or after the occurrence of abnormal glucose metabolism was first discovered a pregnancy-specific disease, Its pathogenesis has not been fully elucidated.This study by measuring the GDM group and NGT group of maternal serum adiponectin,urinary adiponectin and fasting insulin levels, to learn more about GDM women with NGT pregnant women in pregnancy serum, the case of urinary adiponectin levels and insulin resistance and its relevance to further explore the possible development of GDM mechanisms to promote eugenics.Methods: According to the American Diabetes Association published IADPSG criteria selected in December 2014 to December 2015 in Yan'an University Affiliated Hospital maternity clinic or inpatient treatment of GDM was diagnosed 120 cases of pregnant women, to the case group(GDM group), randomly selected in the same period to our hospital, and normal glucose tolerance in pregnant women with 120 cases as normal control group(NGT group) are measured height, weight, all selected pregnant women giving birth, asked in detail about pre-pregnancy weight, weight gain during pregnancy calculated pre-pregnancy body mass index, and asked gravidity, parity, whether there is a family history of diabetes, etc; fasting glucose oxidase colorimetric method using sugar, fasting insulin by radioimmunoassay, ELISA method was fasting BAPN, UAPN; calculate HOMA-IR and third trimester of pregnancy BMI.Result: 1. GDM group in age, gestational age, parity and gravidity pre-pregnancy weight and BMI NGT group differences were not statistically significant(28.9 ± 2.96 years, respectively, 39.5 ± 1.40 Zhou, 1.82 ± 1.03 times, 0.11 ± 0.18 times, 20.9 ± 0.18 kg / m2 and 28.8 ± 3.01 years, 39.1 ± 1.51 Zhou, 1.72 ± 0.58 times, 0.10 ± 0.15 times, 20.5 ± 0.12 kg / m2, P> 0.05). Increased pregnancy weight, BMI and family history of diabetes positive rate of delivery is higher than NGT group, the results have significant difference(respectively 18.95 ± 4.63 kg / m2,31.43 ± 3.71 kg, 30.80%(37)and 15.72 ± 3.52 kg / m2,25.71 ± 3.71 kg, 6.70%( 8), P <0.05).2. GDM group BAPN, UAPN significantly lower than NGT group(5.30 ± 3.03?g / ml, 5.24 ± 1.63 and 12.02 ± 5.89?g / ml, 8.92 ± 1.12?g / ml, P <0.05); and FBG, Fins and HOMA-IR were higher than NGT group, the two groups have significant difference(respectively 5.12 ± 1.27 mmol / L, 15.42 ± 7.90 m IU / L, 3.66 ± 0.32 and 4.25 ± 0.98 mmol / L, 10.81 ± 3.12 m IU / L, 3.27 ± 0.27, P <0.05).3. BAPN levels and other factors correlation: The correlation analysis showed: maternal serum adiponectin levels and HOMA-IR(r =- 0.302, P = 0.002), FBG(r =-0.31, P = 0.001), Fins(r =-0.205, P = 0.037), the third trimester BMI(r =- 0.295, P = 0.047) was significantly negatively correlated with maternal age, gestational age, pre-pregnancy weight BMI, parity, no significant correlation sex(P> 0.05).4. UAPN levels and other factors correlation: The correlation analysis showed: urine of pregnant women adiponectin levels and HOMA-IR(r =- 0.342, P = 0.001), FBG(r =-0.36, P = 0.001), Fins(r =-0.265, P = 0.023), the third trimester BMI(r =- 0.235, P = 0.036) showed a significant negative correlation. And with maternal age, gestational age, pre-pregnancy weight BMI, parity was no significant correlation(P> 0.05).5. GDM as the dependent variable, age, pre-pregnancy BMI, weight gain during pregnancy, late pregnancy BMI, fasting insulin, fasting glucose, blood adiponectin urinary adiponectin and whether family history of diabetes as independent variables row Multiple linear regression analysis the results show hypoadiponectinemia, low urine adiponectin state, childbirth BMI, weight gain during pregnancy and having a family history of diabetes are risk factors for GDM.In conclusion:1. GDM group BAPN, UAPN were significantly lower than NGT group, and weight gain and pregnancy, childbirth BMI, family history of diabetes was negatively correlated.2. GDM group compared with NGT group in addition to the presence of the physiological insulin resistance, but there are still more severe insulin resistance, and is associated with decreased levels of APN.3. hypoadiponectinemia, low urine adiponectin state, childbirth BMI, weight gain during pregnancy and having a family history of diabetes are at high risk factors for GDM.
