Analysis Of Antibacterial Drugs In Clinical Application In Certain Class 3-B Hospital During 2012-2014

Objective By investigating the antimicrobial drugs supply lists between 2012-2014 in a grade-three and second class hospital,the application of antimicrobial drugs of various indicators,adverse drug reactions of antimicrobial drugs,the situation and problems in application,categroies and composition of antimicrobial drugs in this hospital were analyzed according to the requirement of Ministry of Health of the People's Republic of China.The effectiveness of the regulation measures which were taken by the hospital during the special campaigns on antimicrobials from 2011 to 2013 were analyzed and evaluated.The adverse drug reactions of antimicrobial drugs were analyzed to provide the basis for clinical pharmaceutical care.Some problems of antimicrobial application in the key departments were also analyzed to provide the basis for clinical rational use of antibacterial drugs.Methods 1.To analyze whether drug varieties and gauge in this antimicrobial drugs supply directory in this hospital is accorded with national standard,the antimicrobial drugs supply directory in this hospital was compared with regulations of the Ministry of Health of the People's Republic of China.To analyze whether varieties in antimicrobial drugs supply directory in this hospital would ensure the basic needs of clinical application temporary procurement antimicrobial drugs in the hospital were compared with directory drugs.2.Used indicators of antimicrobial drugs in this hospital were extracted to analyze the change tendency of antibiotics utilization ratio and intensity and analyze the effect of three years antimicrobial drugs special regulation in this hospital.3.The data of adverse drugs reactions in this hospital presenting to the National Adverse Drug Reactions Monitoring System from 2012-2014 were extracted to analyze adverse drug reactions of antimicrobial drugs and the correlation between adverse reactions of antimicrobial drugs and drug consumption.4.The rationality in the application of antimicrobial drugs was analyzed by investigating problems of antimicrobial application in ten key departments of clinical,for finding the problems and proposing solutions.Results 1.The number of drug varieties and specifications in the directory selected by the hospital were less than the relevant provisions of the state,and the DDDS of antimicrobial drugs through temporary purchasing in the hospital was accounted for only 0.03% of all.2.The antimicrobial usage in 2012,2013,2014 in this hospital was different significantly and had downward tend,and the intensity of antimicrobial drug use decreased obviously.3.The hospital reported adverse reactions of 607 cases of antibacterial drugs which were involved in 244 cases(40.20%)that female(n = 124)were slightly higher than that of male(n = 120),mainly focus on the ages of 40 and 80 years old(65.57%),and for intravenous drug use(97.13%).The occurrence of adverse reactions were significantly correlated with total antimicrobial drug use frequency(Person coefficient = 0.9999 p < 0.05).4.The unreasonable situation included that in internal medicine department:using antimicrobial drugs without indications;drug selection does not conform to the regulations of Clinical application of antibacterial drugs guiding principles and related managements;time administer drug does not comply with the instructions;long drug treatment;Replace drug without basis or replace frequently.In the medical records of surgical department,?,?,? incision,using antimicrobial drugs without indications;high starting point in drug selection;time administer drug was not reasonable;replacing the drug without basis;using drugs after surgery rather than priorly;long postoperative medication.Conclusion The antimicrobial drug management and application were in line with the relevant provisions of the state in this hospital;The rectification of antibacterial drugs in the hospital has obtained certain achievement.The number of cases adverse reactions involved in the antimicrobial drugs were accounted for the first in all of the reported of this hospital,the occurrence of adverse reactions of antimicrobial drugs was positively correlated with their consumptions.The unreasonable phenomenon was still existed in clinical application of antibacterial drugs in this hospital.They should continue to strengthen the management and training of antibacterial drugs.
