Aim Investigate the efficacy of pronase and simethicone,which were applicated as premedication for gastroscopy.To provide the evidence for the necessity of combined pronase and simethicone as medication before gastroscopy.Methods Subjects were enrolled from the People's Hospital of Yuanzhou County from April 2016 to December 2016.655 outpatients who underwent gastroscopy in this hospital were randomly assigned to receive one of two premedications as follows: simethicone,pronase and sodium bicarbonate with 20 minutes premedication times,lidocaine hydrochloride mucilage with 10 minutes premedication times(study group).Only lidocaine hydrochloride mucilage with 10 minutes premedication times(control group).The primary endpoint was the detection rates of early gastric cancer.The secondary endpoints were adverse reactions of patients,the visibility score of gastroscopy,the flushing times and time of examination,the positive detection rates of micro-lesions.Endpoints were observed in the two groups respectively,and the data was analyzed by SPSS 21.0 software.Results The study included a total 655 patients,among them,5 cases for the shedding.Finally,650 patients were enrolled in this study final,322 cases in study group and 328 cases in control group.In this study,a small number of patients with mild or moderate nausea and discomfort after premedication,there was no significant difference between study group and control group(P =0.124).The average visibility scores of gastroscopy were 5.72 ± 1.25 in study group and 6.78 ± 2.25 in control group,the visibility scores of study group were significantly lower compared with those of control group,that means,the vision clarity of study group is better than that of control group(P <0.001),Study group had the lowestfrequency of flushing,which was significantly lower than that of control group(P <0.001).The average operation time of gastroscopy in the study group was 176.15 ± 28.92 s and that was 181.84 ± 50.83 s in the control group,there was no significant difference between the two groups(P =0.078).The detection rate of ulcer and bleeding points in study group was 12.7 %and 3.1 %,respectively,in control group was 7.3 % and 0.6 %,the detection rates of study group were higher those of control group(P =0.021,P =0.018).In study group,the case of early gastric cancer was significantly higher than that of control group(P =0.007),7 cases of early gastric cancer were detected in the study group,and no case of control group which was diagnosed with early gastric cancer.Conclusion The application of pronase and simethicone as premedication for gastocopy,that can remove foam and mucus on the gastric mucosa effectively,and to improve visibility of vision.It is helpful for the detection of small lesions and early gastric cancer.
