The Clinical Significance Of Gastroscope In Healthy Population Screening Of Early Gastric Cancer And Precancerous Diseases

Objective Understand healthy people incidence trends and characteristics of early gastric cancer and precancerous disease,to explore the importance of healthy people line of gastroscopy,also for the future arrangements for screening of early gastric cancer and precancerous disease to provide clinical basis.Methods Analysis 1281 cases for healthy gastroscopy examination in our hospital(organizational unit check-up crowd,high-end people and some special groups)of gastroscope and pathology results during September 2015-November 2017,and at the same time period,1281 cases with the digestive tract symptoms to digest the outpatient service line gastroscopy patients as controls.Results The detection rate of chronic atrophic gastritis,gastric ulcer,gastric polyps and gastric cancer in health group is 5.62%,5.85%,5.62% and 5.85% respectively,including 2 early gastric cancer.Outpatient group check out the corresponding disease rate of 9.84%,8.04%,8.82% and 8.04% respectively,including1 early gastric cancer.Two groups of disease detection difference besides gastric polyps have statistical significance;Two groups of gastric ulcer,gastric cancer are predominantly male,gastric polyps in the majority with women.Chronic atrophic gastritis in health group are given priority to men,but in the majority with women in the outpatient group(P<0.05);Chronic atrophic gastritis,gastric ulcer checked out higher detection rate are with the incremental age,the detection of gastric polyps and gastric cancer have no obvious statistical significance with the age,but polyps detection rate show a trend of increase with age.Disease in age group differences in two groups is statistically significant : chronic atrophic gastritis in 40-50,50 to 60 years old,and gastric ulcer in over 60 years old.The peak age of gastric polyp in two groups is 51 to 60 years old and over 60 years old,and there is no statistically significant difference among the different age groups.A high incidence of gastric cancer in people over 60 years old,along with the age growth higher detection rate;Hp infection rates are 39.73% and 43.79% respectively in health and outpatient group(P<0.05),man is the highest detection rate.As the growth of the age,Hp infection rate showed a trend of rise.Disease of Hp infection rate from high to low are gastric cancer,gastric ulcer,the chronic withering gastritis,gastric polyp.The relationship between disease and Hp infection has no obvious difference between the two groups(P>0.05),and the relationship between Hp infection and various diseases in each group is statistically significant;Gastric ulcer obliterans occur gastric body(34.67%),gastric antrum(30.67%)and the Angle of the stomach(16%)in health group.Gastric ulcer obliterans occur gastric antrum(32.04%),stomach Angle(23.30%)and place(25.24%)in outpatient group.The differences in two groups of ulcers,are multiple parts and gastric body,which have significant statistical significance(P<0.05).105 cases(8.2%)of gastric polyps in health group,including fundic gland polyps(70.48%),hyperplastic polyp(18.10%),inflammatory polyps(9.52%),adenomatous polyps(1.90%);Outpatient group check out 113 cases(8.82%)gastric polyps,and corresponding polyps pathological type is 63.72%,22.12%,13.27%,0.88% respectively.Fundic gland polyps is the mainl part in two groups of detected polyps,there is no statistically significant difference for various types of polyps(P>0.05);The difference in degree of inflammation and atrophy of chronic atrophic gastritis was statistically significant,the degree in health group light outpatient group obviously.And there is no statistical difference in degree of inflammatory activity and intestinal meaning;the difference among two groups of gastric inflammation and activity degree is statistically significant,outpatient service group have heavier degree.there is no statistically significant difference.Conclusion 1.The incidence of gastric cancer and precancerous disease with older than 40 male of healthy physical examination people is relatively high.It is necessary to take the gastroscope examination.2.Healthy people gastroscope examination can significantly improve the detection rate of early gastric cancer,biopsy for pathological degree of disease and prognosis assessment is of great significance.3.Hp infection rate showed a trend of rise with age,male susceptible;there is a close relationship between the occurrence of gastric cancer.
