Objective: Three dimensional finite element analysis was used to find the difference between the new screws with holes and the traditional screws. To find out the way of using the new screws to enhance the stabilization of the internal fixators, reduce the postoperative complications and the mortality of periprosthetic femoral fractures. The disadvantage could be found out,and some measures should be taken to improve it.Methods: The femoral data of an old man was obtained by CT,with which the 3D model could be reconducted in the Minics software. And then, the data of the hip prosthesis, the locking plate, the screws and the cables was put into the MINCS to rebuild the 3D models. All the models were exported to the solidworks software. The different Vancouver B1 fracture models were conducted with locking plate, screws and cables, according to the regular orthopaedic surgical procedures. And,the fracture line was located at about1.5cm far from the tip of the hip prosthesis. The data of the models was imported into the ANSYS software, and faced the same condition. The stress map and displacement were analyzed by the ANSYS V12.1 software. Results: The different models had the similar stress map and max stress, while they were in the same condition. As to the fracture, the model A with the traditional screws had to face the more small stress than the model B with the new screws;on the contrary,it had the lager displacement than model B.Taking a glance at the locking plate, the stress on the model A is more heavy than model B.Conclusion: Using the three dimensional finite element analysis,it is can be find that the new screws model can firm the internal fixation, reduce the displacement of the fracture and increase the stress on the fracture. And then,it can decrease the postoperative complications. On the second hand, since it can decrease the stress of the locking plate, using the new screws can also reduce the appearance of the fracture of the locking plate, and enhance the stability of the internal fixtation.
