Objective: To analyse the clinicopathological characteristics,prognosis and recurrence of granulosa cell tumour of the ovary(GCT).To study the factors influencing the relapse and prognosis of GCT.Methods:1 Various clinicopathologic factors of 112 patients with granulosa cell tumour admitted to the Fourth Hospital of Hebei Medical University between February 2005 and September 2016 were reviewed.2 The effects of clinicopathological characteristics upon recurrence were analyzed by SPSS21.0.Results:1 General condition: There are total 112 patients,adult granulosa cell tumor(AGCT)are 107 cases,and juvenile granulosa cell tumor(JGCT)are 5 cases.The patients' average age was 46.50±13.93(12-75 years old)years old.The average level on preoperative serum CA125 was 118.11±415.61 IU,The percent of elevated level on preoperative serum CA125 was 38.50%(35/91).2 Surgery:29(25.90%)cases received conservative surgery included 16(55.20%)cases with unilateral or bilateral ovarian cystectomy and 13(44.80%)cases with unilateral oophorectomy,47(42.00%)cases underwent comprehensive surgical staging.There were 37(33.00%)patients operated by the way of laparoscopic surgery,75(67.00%)patients operated by the way of laparotomy surgery.3 Adjuvant chemotherapy: 83(74.20%)cases had postoperative adjuvant chemotherapy,the regimens of which were mainly a platinum-based intravenous chemotherapy,the regimens such as BEP,VAC,TP,TC and so on.4 Pathological features: Mean size of the tumors was 9.25±5.45(1.60-48.00cm)cm.The bilateral tumor is 2 cases(1.8%),the unilateral tumor is 110 cases(98.20%).The 112 patients were used the latest version of the FIGO stage,and the results were shown in the stages: 105 cases?,4 cases II,3 cases III,the lymph nodes are negative.5 Recurrence: Among 112 patients,there were 105(93.80%)cases in stage ?,4(3.60%)cases in stage ? and 3(2.70%)cases in stage ? respectively.Mean duration for follow-up was 52.63±37.82 months,and 11(9.80%)patients had recurrence.The average months of recurrence was 83.09±59.92(22-228 months)months.6 Survival analysis :To the deadline,total 7(6.30%)cases tumor-related death were reported,6 cases died within 5 years and 7 cases died within 10 years,42 cases were followed up for 5 years,and 15 cases were followed up for 10 years,and 1 case was stage ?.The 5-year survival rate for stage ? was 85.71%,and the 10-year survival rate was 57.14%.Conclusions:1 The level of serum CA125 cannot be used as indicators of identification for AGCT or JGCT,CA125 of ascites group elevated compared with the group of without ascites.2 Tumor recurrence shortened the progression-free survival for 5 years.3 FIGO stage of patients with GCT influenced tumor recurrence,later stage is more likely to relapse.4 The patients of stage ? with chemotherapy can prolong survival in patients with high-risk factors.5 Recurrent patients with a satisfactory tumor cell and chemotherapy can prolong survival.
