Research On The Application Of Autograft Ossicle And Titanium Ossicle Prosthesis In Tympanoplasty

Objective: To investigate the short-term efficacy of autograftossicle and titanium ossicleprosthesis intype ?andtype ?tympanoplasty.Methods:117 patients(120 ears)who receivedtympanoplasty in Shenzhen Longgang District Otorhinolaryngology Hospital from January 2014 to December 2015 were selected for retrospective study.Among the 120 ears,60 ones(30 ones for autograftossicle andtitanium ossicleprosthesisrespectively)received type II tympanoplasty,and 60 ones(30 ones for autograftossicle andtitanium ossicleprosthesisrespectively)received type III tympanoplasty.The patients using autograftossicle andtitanium ossicleprosthesis in different tympanoplasties were divided.The patients' gender,ear,age,middle ear risk index(MERI)and disease constitutionwere compared.Besides,the patients had preoperative and postoperative 6 months ear endoscope inspection and pure tone audiometry testing.The statistical analysis was conducted on their tympanic membrane healing and postoperative hearing recovery.Results:.1.For thetype II and type III tympanoplasty,no differences showed between autograftossicle andtitanium ossicleprosthesis group(P>0.05)in terms ofthecomparison results of gender(male/female),ear side(left/right),age,MERI and disease(chronic suppurative otitis media/middle ear cholesteatoma)constitution.2.In the type II and III tympanoplasty,operation time in autograftossicle was longer than titanium ossicleprosthesisgroup(P<0.05).3.The comparison of the tympanic membrane healing of autograftossicle andtitanium ossicleprosthesis group in thetype II and III tympanoplastydidn't make any difference(P> 0.05).4.There were not any differences betweenthe autograftossicle andtitanium ossicleprosthesis group in type II tympanoplasty in terms of preoperative gas conduction value,bone conduction value,postoperative gas value and air difference(P>0.05).There were not any differences betweenthe autograftossicle andtitanium ossicleprosthesis group in type III tympanoplastyin terms of preoperative gas conduction value,bone conduction value and postoperative bone value,but had statistically significant differences in terms of postoperative gas value and air difference(P<0.05).The average gas conduction value of autograftossicle andtitanium ossicleprosthesis group increased from 60.5±12.0d Bto 37.0±14.5d B,and from 58.5±13.2d Bto 28.2±5.9d Brespectively.5.For the effective rate comparison of autograftossicle andtitanium ossicleprosthesis group,the differences didn't showed any difference(P>0.05)in type II tympanoplasty,while they were statistically significant(P<0.05)in type III tympanoplasty.In terms of the comparison of postoperative air-bone gap(ABG),the differences of autograftossicle andtitanium ossicleprosthesis group were not obvious(P>0.05)in type II tympanoplasty,while they were statistically significantin type IIItympanoplasty(P <0.05).Conclusion:1.In the topic tympanoplasties,autograftossicle group takes longer operation time than titanium ossicleprosthesis group.2.Autograftossicle group and titanium ossicleprosthesis group has better short-term recovery of tympanic membrane after type ?and ? after tympanoplasty;patients' gas conduction and bone conduction hearing improved;in type ? tympanoplasty,titanium ossicleprosthesis group improves more obviously than autograftossicle group.3.After type II tympanoplasty,there isnot significant difference in the curative effect of autograftossicle group and titanium ossicleprosthesis group;after type III tympanoplasty,titanium ossicleprosthesis group has better curative effect than autograftossicle group.
