Object:1.Based on the framework of timing theory,through qualitative interview to understand the care experience of stroke patients in different stages of the disease,analyze the dynamic changes of their care needs and quality of life,and put forward targeted suggestions2.Formulate a "5T" stage nursing intervention plan based on timing theory,and explore the impact of this intervention plan on the caregiver's needs,quality of life,and patient self-care ability.Method: 1.The first part is qualitative research: under the guidance of the timing theory,the intention sampling method is adopted to select the caregivers of a stroke patient in neurology department as the research object.The real care experience of 48 caregivers in five stages is understood through semi-structured interview.The data are analyzed by Lo Biondo wood phenomenology to understand their care needs and quality of life in different periods.2.The second part is the intervention study: 89 pairs of stroke patients and their caregivers in a neurology department were selected as the study object by convenient sampling method,45 pairs in the control group and 44 pairs in the intervention group.Taking the caregiver as the intervention object,the control group was given routine care,and the intervention group received the "5T" stage nursing intervention program(under the guidance of the timing theory,the intervention program was developed around the characteristics of the caregiver's needs in five stages),taking the satisfaction of the caregiver's needs,the quality of life and the patient's self-care ability as the evaluation indicators,respectively at the time of discharge Three months after discharge and six months after discharge.Spss22.0 software was used for data entry and analysis.Result:1.5 stages of caring experience are extracted:(1)Disease diagnosis period:(1)unacceptable to reality(2)life rhythm is disrupted(3)emotional support needs(4)disease information needs;(2)Stable period:(1)freedom and livelihood constraints(2)economic stress(3)rehabilitation training guidance(4)care skills needs;(3)Preparation period:(1)the need for psychological stress adjustment;(2)take care of strength challenges;(3)positive experience;(4)Follow-up effective information and planning needs;(4)Adjustment period:(1)decline in physical health(2)need for emotional grooming(3)need to help home adjustment(4)need for home care(5)need for group mutual assistance;(5)adaptation period:(1)The need for patients to return to society(3)The need for effective information continuation(4)The need to improve the quality of life;2.(1)there was no significant difference in the basic data between the two groups before intervention(p>0.05);(2)after intervention,the scores of demand satisfaction in the intervention group were better than those in the control group at each test time point,and the difference was statistically significant(p<0.05),and there was an interaction phenomenon(P<0.01);The demand of intervention group was higher than that of control group.(3)the scores of self-care ability of the intervention group were better than those of the control group at all time points after intervention.The difference was statistically significant(p<0.05),and there was interaction phenomenon(p<0.01);The increasing trend of self-care ability in the intervention group was greater than that in the control group.(4)after intervention,the scores of all dimensions of life quality of intervention group were better than those of the control group at each test time point,and there was a significant difference between 3months and 6 months after discharge(p<0.05),and there was an interaction phenomenon(p<0.01);The quality of life in the intervention group was higher than that in the control group.Conclusion: 1.Qualitative research results show that caregivers' experience of caring at different stages of the disease is dynamic,with different needs for care,and factors affecting the quality of life of caregivers at different stages of the disease.2.On the basis of qualitative research,formulating a "5T" stage nursing intervention plan based on timing theory has a significant effect,which can effectively meet the care needs of caregivers,significantly improve the quality of life,and also improve patients' self-care ability.
