Objective:To analyze the relative risk factors of anastomotic leakage after rectal cancer surgery.Methods:Clinicopathological data of 734 rectal cancer patients undergoing Total Mesorectal Excision surgery from January 2010 to September 2013 in our hospital was retrospectively analyzed. 2? test and Fisher’s exact test were performed for univariate analysis, and Logistic regression test was carried out for multivariate correlation analysis.Results:The incidence rate of anastomotic leakage was 6.53%(48/734); Univariate analysis showed that the occurrence of anastomotic leakage was correlated to sex,diabetes, distance of the tumor from the anal verge(DTAV),neoadjuvant chemotherapy, defunctioning stoma, postoperative hypoalbuminemia, and blood vessel invasion(P<0.05).Meanwhile anastomotic leakage was showed no association with age, nationality, hypertension, preoperative hypoalbuminemia, postoperative D-Dimer, white blood cell(WBC), carcino-embryonic antigen, American Society of Anesthesiologists classification, Tumor Lymph Metastasis stage, tumor differentiation degree, lymph node metastasis, operation mode, operation time, Bleeding volume, body mass index, nerve invasion(P>0.05); Multivariate analysis showed that the occurrence of anastomotic leakage was closely correlated to sex, DTAV, neoadjuvant chemotherapy, defunctioning stoma, and blood vessel invasion(P<0.05).Conclusion:Sex, diabetes, DTAV, neoadjuvant chemotherapy, defunctioning stoma, postoperative hypoalbuminemia, and blood vessel invasion could influence the occurrence of anastomotic leakage; sex,DTAV,neoadjuvant chemotherapy, defunctioning stoma, and blood vessel invasion were the independent risk factors of postoperative anastomotic fistula,the related risk factors shoud be taken into consideration in clinical to formulate the effective treatment scheme,which will play important roles in reducing the occurrence of postoperative anastomotic leakage.
