Objective:Discuss the serum amylase for the value and reliability for diagnosing chronic kidney disease patients accompany with acute pancreatitis.Methods:We retrospectively evaluated the researched cases of the first and second affiliated hospital of shanxi medical university and Shanxi Province people's hospital in January 2010 to December 2016.Inclusive criteri:1?Acute pancreatitis in patients with chronic kidney disease;2?Patients with chronic kidney disease complicated with abdominal pain or abdominal discomfort;3 ? Acute pancreatitis in patients without chronic kidney disease;4 ? All cases were diagnosed by abdominal ultrasound or abdominal CT examination or exclude the diagnosis of acute pancreatitis;5?Patients with chronic kidney disease have been diagnosed before admission.The cases were divided into CKD+AP group,CKD group and AP group.Results:In CKD + AP group and AP group comparison,two groups of serum amylase leels were(655.7414.3)and(655.7299.4),the sensitivity of diagnosis were 77.8% and 85.7% respectively,no statistically significant difference between two groups of data.CKD group of serum amylase was(246116.4),compared with the CKD+AP group,there was statistical difference?According to the data of two groups,the specificity of serum amylase was 81.6%.If the specificity was raised,it was 4 times higher than the reference value as the standard,the specificity could be increased to 93.9%,but the sensitivity was reduced to 61.1%,and the missed diagnosis rate was 38.9%.The correlation between AMY and CREA in patients with CKD was compared.The correlation coefficient was 0.769.Conclusion:Serum amylase to the diagnosis value of chronic kidney disease with acute pancreatitis is limited,especially renal damage is relatively serious,patients with higher serum creatinine.
