The Clinicalanalysis Of Paragonimiasis In 1174 Childern

Objective: To investigate the clinical feature and treatment of paragonimiasis in children,in order to improve clinicians understanding of paragonimiasis,so as to reduce the misdiagnosis rate,and to provide clinical reference for the diagnosis and treatment of paragonimiasis.Methods : A total of 1174 cases with paragonimiasis in Children's Hospital of Chongqing Medical University from June 1991 to December2015 were enrolled in our study.The clinical data,including epidemic history,clinical manifestation,laboratory examination,imaging findings and treatment of paragonimiasis,were retrospectively analyzed.Result: Male children were the main component in children with paragonimiasis.The vast majority of children aged between 3 to 10 years old.72.06% of the patients had clear history of epidemiology,and eating raw crab or drinking stream water was the most common.The clinical manifestations were complex and diverse.Lung type(34.41%)was the most common,followed by mixed type(33.65%),brain type(13.54%),skin type(13.03%)and abdominal type(2.13%).The imaging findings of paragonimiasis were varied,85.65% of the patients had abnormal chestX-ray film,and 97.91% had abnormal chest CT.Only 2.54% of the biopsy showed worms or eggs.Moreover,74.91% of eosinophil were increased.68.73% of the children were misdiagnosed in other hospitals.A total of1164 patients were given praziquantel insecticide treatment.Most of the patients were cured or improved,and 2 cases died.Conclusion:The clinical manifestations of paragonimiasis are lack specificity,which is easy to be misdiagnosed.Therefore,the diagnosis should be combined with epidemiological history,clinical manifestation and auxiliary examination.After the diagnosis,praziquantel should be used as anthelmintic treatment,and surgical treatment could be performed when necessary.Moreover,the majority of children with paragonimiasis have a good prognosis,however,a few can leave serious sequelae and even death.
