Objective This paper retrospective analysis 26 cases of primary oral mucosal melanoma of oral and maxillofacial surgery in the Affiliated Hospital of Qingdao University from January 2000 to December 2015,mainly describes the characteristics of the cases and survival data of primary oral mucosal melanoma patients in our hospital by using the method of statistical description,to access the outcome and identify clinical prognostic factors in a recent cohort of patients with primary oral mucosal melanoma.Provide case data and data support for the diagnosis,treatment and prognosis of primary oral mucosal melanoma in our hospital andother domestic hospitals,help to improve the understanding of this disease and the level of diagnosis and treatment,improve the prognosis of patients.Materials and methods Collect the clinical data of 26 cases of primary oral mucosal melanoma patients in oral and maxillofacial surgery,Affiliated Hospital of Qingdao University in recent 16 years all patients were diagnosed by histopathology.Induce and analysis the clinical characteristics of 26 cases of primary oral mucosal melanoma patients,and find out the relationship between gender age,tumor stage,systemic therapy and the prognosis of the disease,all treated patients were followed up for overall survival and 3 year,5 year survival rates and the median survival period were estimated in the treated patients.All statistical analyses were performed using SPSS 19.0 software package,and P <0.05 was used as the standard.Postoperative survival period analysis were computed by the Kaplan-Meier method and compared by the Log-rank test,the difference was statistically significant with P <0.05.Results The Affiliated Hospital of Qingdao University Department of oral and maxillofacial surgery admitted to hospital of the primary oral mucosal melanoma patients with a total of 26 cases,including 15 cases of male,11 cases of female,the ratio of male to female was 1.36:1.The overall age of onset was about 40 years old,the peak age of onset was 50~70 years old,the median age of onset was 53.Origin site of 26 cases of primary oral mucosal melanoma patients,12 cases of palate(46.15%),11 cases of gingival(42.31%),and 3 cases of buccal mucosa(11.54%).There were 8 patients with stage III and 16 patients with stage IVA,the ratio was 1:2,a total of 2 patients gave up treatment.16 cases of patients with cervical lymph node metastasis in all of the 26 cases,the rate of metastasis was 61.54%.In 4 of 24 patients were treated with simple surgery(2 cases)or cryotherapy(2 cases),and the recurrence rate was 50% and 100%;patients with surgery plus cryotherapy a total of 6,the recurrence rate was 50%;with surgery plus cryotherapy plus neck dissection a total of 10,the recurrence rate was 30%;with surgery plus cryotherapy plus neck dissection plus chemotherapy plus biological immune therapy a total of 4 people,at the end of the follow-up time 4 patients had no tumor recurrence,cervical lymph node metastasis and distant metastasis,and the average survival time of these 4 patients was 8 years and 10 months.The 3 year and 5 year survival rates of the patients were 41.7% and 25%,median survival period was 30 months,the 3 year and the 5 year survival rates of patients with stage III and IVA were 25% and 25%,respectively,with the rate of 50% and 25%.Gender(male and female),age(over 56 years and less than 56 years old)and clinical stage(III and IVA)have no correlation with the prognosis of the primary oral mucosal melanoma(P>0.05),there was statistically significant difference between different systemic therapy(surgery plus cryotherapy and surgery plus cryotherapy plus neck dissection plus chemotherapy plus biological immune therapy),namely there was a correlation between systemic therapy and prognosis(P <0.05).Conclusion 1.In Affiliated Hospital of Qingdao University,the male more than the female with primary oral mucosal melanoma diagonsed,the ratio is 1.36:1,the high incidence in all of patients was 50~70 years old,The most common site of the primary tumor is palatal mucosa and gingival mucosa.2.In Affiliated Hospital of Qingdao University,the oral mucosa of primary melanoma with stage IVA(16 cases)more than III(8 cases),the ratio is 2:1.Patients who surgery plus cryotherapy plus neck dissection plus chemotherapy plus biological immune therapy were better than those of patients who surgery plus cryotherapy plus neck dissection in survival rate,the local recurrence,lymph node metastasis and distant metastasis.3.The 3 years and 5 years survival rate of 24 primary oral mucosal melanoma are about 41.7% and 25%,the median survival was 30 months for the 24 patients.4.The 24 patients' gender,age and tumor stage have no significant difference in the prognosis of primary oral mucosal melanoma.There was significant difference between the different systemic therapy.
