Objective Using multiple joints isokinetic testing and trainning instrument to observe the effect of isokinetic trunk muscle training and core muscle strength training on rehabilitation therapy in patients with chronic lumbar intervertebral disc protrusion.To evaluate different effects of muscle function and proprioception in patients with chronic lumbar disk herniation using isokinetic trunk muscle strength training and core muscle training and to provide theoretical basis and guidance for the rehabilitation for patients with chronic lumbar disk herniation.Methods Eighty patients were randomly divided into treatment and control groups.All patients are given regular routine rehabilitation therapy,including interference with physical factors,such as electric,microwave,pelvic traction treatment.Treatment group were given isokinetic trunk muscle strength training on the basis of routine rehabilitation treatment,The training mode was the common uniform centripetal/centripetal pattern,continuous treatment for 8 weeks,treatment 3 times a week.The control group were given core muscles trainingon on the basis of routine rehabilitation treatment,continuous treatment for 8 weeks,treatment 3times a week.Two groups of patients were using isokinetic testing system to test the trunk muscle function before and after treatment respectively,evaluation indicators include: peak torque(PT),relative peak torque(PT/BW),total work(TW),and average power(AP),expand ratio(F/E);Respectively before and after treatment using standard Japanese orthopedic association low back pain score(Japanese orthopaedic association,JOA),Oswestry lumbar dysfunction index scale(ODI),visual analogue scale(visual analoguc scale,VAS)to evaluate patients function to improve the situation;Usingisokinetic trunk muscle strength tester proprioception test mode to evaluate before and after treatment in patients with ontology sense.which were given isokinetic trunk muscle strength and core muscle training respectively lasted for 8 weeks,three times per week.Peak torque(PT),relative peak torque(PT/BW),total work(TW),and average power(AP),The ratio of flexion and extension(F/E);lower back pain standard scoring criteria of Japanese Orthopedic Association(JOA)、Oswestry disability index(ODI)、visual analogue scale(VAS).Lumbar proprioception assessment were used to evaluate patients before and after treatment.Results After eight weeks training,the assessment of available: trunk muscle strength evaluation indexes from two groups patients before treatment were increased significantly(P <0.05),compared with the control group,treatment group improved more significantly(P <0.05);Two ratio of F/E of two groups with flexion and extension increase gradually with angular velocity increases,the treatment group F/E ratio under 60 ° / s earlier significantly decreased(P < 0.05),but under 120 ° / s 、180 ° / s two kinds of angular velocity change is not obvious(P > 0.05),the control group in 60 ° / s 、120 ° / s、180 ° / s three angular velocity,the ratio of F/E was no statistically significant difference before and after treatment(P > 0.05);The score test of JOA,ODI,VAS suggest both treatment in patients with lumbar function can be significantly improved(P < 0.05),but the effect of treatment group is more obvious(P <0.05);For proprioception test results show that the two treatments had obvious change after treatment(P < 0.05),and more obvious tendency in treatment group than the control group(P<0.05).Conclusions Comparing with core muscle training,isokinetic trunk muscle strength training could improve muscle function 、clinical function and proprioception obviously in patients with chronic lumbar disk herniation,which is the new choice of rehabilitation therapy in patients with chronic lumbar intervertebral disc protrusion. |
