Objective To investigate and analyze the situation of quality of life and the influencing factors of pulmonary rehabilitation exercise of chronic obstructive pulmonary disease patients.To analyze the relationship between quality of life and degree of dyspnea,exercise tolerance,and pulmonary function.To explore the application effect of trans-theoretical model of pulmonary rehabilitation exercise on COPD patients.To seek scientific and effective pulmonary rehabilitation exercise interventions for COPD patients and to provide theoretical guidance and references for improving the behavior of pulmonary rehabilitation exercise.Method 98 cases of COPD patients who met the inclusion criteria from January to December in 2015 were recruited from Respiratory Medicine of Ningxia People's Hospital.Those subjects were randomly divided into the treatment group and control group(49 in each group).The patients in treatment group were received pulmonary rehabilitation exercise intervention based on TTM and normal pulmonary rehabilitation exercise intervention was given to patients in the control group,and the interventions for both group were implemented from March to November in 2016.Behavior changes,frequency of pulmonary rehabilitation exercise,quality of life,degree of dyspnea,walking distance within six minutes,and pulmonary function of patients with COPD were assessed before,at 3 month and 6 month during the intervention and after finishing the intervention,and then effects of two groups were compared after those interventions finished.Results1.A toal of 94 subjects were enrolled and completed in this study,among them 58(61.7%)cases were male and 36(38.3%)cases were female,aging(68.94±10.76)on average;the mean disease duration was(14.45±6.41)years;the mean frequency of hospitalizations due to COPD in one year was(2.55±1.12)times.2.The mean score of CAT was(24.00±7.44),mean score of MMRC was(2.81±1.08),mean distance of 6MWD was(330.24±69.98)meters,the ratio of FEV1/FVC was(55.74±10.11)% and FEV1%pred was(54.68±10.20)%.3.Before the interventions,the stage of pulmonary rehabilitation exercise compliance behavior change in COPD patients,of which people in previous intention were most and the proportion was 46.8%,of which people in preparatory stage were least and the proportion was7.40%.4.The significant influencing factors regarding COPD patients' quality of life are the number of hospitalizations due to COPD in one year,knowledge and implementation of pulmonary rehabilitation exercise(P<0.05).5.The score of CAT was positively correlated to MMRC(r = 0.937,P<0.05),which was negatively correlated to 6MWD(r =-0.981,P<0.05)and pulmonary function(r =-0.933,P<0.05).6.Before the interventions,there were no significant differences between the treatment group and the control group in demographic characteristics about clinical data,pulmonary rehabilitation exercise behavior,CAT score,MMRC score,distance of 6MWD,FEV1/FVC and FEV1%pred(P>0.05).7.After the interventions,the change of pulmonary rehabilitation exercise of the treatment group was better than that of control group(P<0.05),and the exercise frequency of pulmonary rehabilitation was higher than that of control group with significant difference(P<0.05).8.After the interventions during six and nine months,the degree of dyspnea of treatment group was lower than that of control group(P<0.05),and the quality of life and exercise tolerance of treatment group were better than control group(P<0.05).9.After the interventions during nine months,pulmonary function of test group was better than that of control group,and FEV1/FVC,FEV1%pred were higher than control group(P<0.05).Conclusions1.The number of hospitalizations due to COPD during one year,knowledge and implementation of pulmonary rehabilitation exercise can affect quality of life of patients with COPD.2.The score of CAT was relevant to MMRC,6MWD and pulmonary function.We should comprehensively assess the quality of life of patients by monitoring the indicators.3.Pulmonary rehabilitation exercise interventions based on trans-theoretical can effectively improve the exercise compliance.4.Pulmonary rehabilitation exercise interventions based on trans-theoretical effectively relieve the patient's breathing difficulties,enhance athletic endurance,strengthen pulmonary function,and improve patients' quality of life.
