Objective:This article attempts to apply the Distress Thermometer in different period of schizophrenia,making sense of the level of psychological distress of patients and the occurrence of significant distress,and to explore the sources of distress in different period of schizophrenia,providing a more simple and convenient method to screen psychological distress of schizophrenia and make references for prevention and treatment of schizophrenia.Methods: This research is divided into two parts.The first part: According to the general information form,Distress Thermometer,Positive And Negative Syndrome Scale,Hospital Anxiety Depression Scale,Activity of Daily living scale,uses random sampling method to collect 100 cases of schizophrenia patients' general information of a mental health center of Shaanxi Province,evaluating the degree of distress,mental symptoms,the degree of anxiety and depression,self-care ability of the patients and analyzing the probable sources and risk factors of psychological distress of patients with schizophrenia in acute stage.Using statistical software SPSS19.0,the first part will analyze the data of patients by Chi-square test,rank sum test,Mc Nemar test,logistic regression analysis,roc curve and spearman correlation analysis.The second part: from 71 patients who are still in hospital among 100 cases of schizophrenia patients in the first study after 6 weeks of regular hospital treatment,the second research chooses 64 cases of patients who have been in recovery period to be evaluated on the degree of distress,mental symptoms,the degree of anxiety and depression and clinical efficacy by distress thermometer,positive and negative syndrome scale,hospital anxiety depression and clinical global impression,analyzing sources and influencing factors of patients with psychological distress in recovery period by using SPSS19.0 statistical software for data analysis.Result :1.The incidence of significant psychological distress in patients with acute schizophrenia was 57%,and the score of DT was 4.86 ± 2.38.By using the hospital anxiety and depression scale(HADS)score? 18 is divided into judging whether there is significant psychological distress,the optimal critical value of psychological pain was 5,the area of ROC curve was 0.751(p < 0.01),Youden index was 0.503,sensitivity was 74.6%,specificity was 75.7%;The score of Distress Thermometer in patients with acute schizophrenia was positively correlated with the score of the Hospital Anxiety and Depression Scale;Among them,the Problem List(PL)in the practical problems of care for the elderly and children,work and school;the emotion problems of depression,loneliness,anxiety,worry,lack of interest in life,sleep,memory and attention decline;physical problems in eating,fatigue are affecting the degree of psychological distress of patients with risk factors(P <0.05);The general information,positive and negative symptoms,activity of daily living is not statistically related with the significant psychological distress in patients.2.The incidence of significant psychological distress in patients with convalescent schizophrenia was 31.3%,and the score of DT was 3.34±1.748;By using the hospital anxiety and depression scale(HADS)score? 18 is divided into judging whether there is significant psychological distress,the optimal critical value of psychological pain was 5,the area of ROC curve was 0.703(p < 0.01),Youden index was 0.503,sensitivity was 61.1%,specificity was 82.6%;The Problem List(PL)in the practical problems of care for the elderly and children,economic problems,environmental problems;interpersonal problems in associating with relatives and friends;the emotion problems of depression,fear,loneliness,anxiety,worry,memory andattention decline are the factors that affect the psychological distress of patients(P <0.05).The scores of Hospital Anxiety Depression Scale in patients were significantly correlated with the degree of psychological distress(P <0.05);The scores of Clinical Global Impression scale is not statistically related with the significant psychological distress in patients.Conclusion:1.Ditress Thermometer can be used in the screening of psychological ditress in patients with different stages of disease,and provide a convenient and effective reference for clinical staff in time to understand the inner pain and its influencing factors.;2.The Cut-off point of DT for the screening of psychological ditress in patients with acute and convalescent schizophrenia was 5 points,the sensitivity were 74.6 % and 61.1%,the specificity were 75.7 % and 82.6%;3.The incidence of psychological ditress in patients with acute schizophrenia was 57%.Care for the elderly and children,work and school in the practical problems,Depression,loneliness,tension,anxiety,loss of interest in life,sleep,memory / attention deficit in emotional problems;The eating and fatigue of body problems are the risk factors to affect the degree of psychological distress of patients;4.The incidence of psychological ditress in the patients with schizophrenia was 31.3%,including the elderly and children,economic problems,surrounding environment,with relatives and friends,depression,fear,loneliness,tension,anxiety,memory and attention decline were the main factors affecting the psychological distress of patients;5.The incidence of significant psychological distress in patients with acute schizophrenia was 57%,and the recovery rate was 31.3%.This indicated that the score of Distress Thermometer changed with the change of the condition.The score of Distress Thermometer can also be used as an indicator of the severity of the disease.
