Objective: To compare via sinus tarsi approach combined with universal locking plate and the traditional "L" shaped incision combined with plate fixation for the treatment of SandersⅢ calcaneal fracture treatment effect of single axial locking.Methods: Since this study from January 2010 to December 2015,SandersⅢ total of 60 cases of calcaneal fracture patients were randomly divided into observation group and control group of 30 cases The observation group had 25 cases of male patients and 5 female patients,aged between 24-55 years old,the average age of 34.6 years,of which high fall injury in 27 cases,3 cases of traffic accident,12 cases of type SandersⅢAB,type SandersⅢAC 12 cases,6 cases of type SandersⅢBC.The control group had 26 cases of male patients and 4 female patients,aged between 22-58 years old,the average age is 35.7,of which high fall injury in 26 cases,4 cases of traffic accident injury,13 cases of type SandersⅢAB,type SandersⅢAC 11 cases,6 cases of type SandersⅢBC.The observation group was treated by tarsal sinus gap minimally invasive incision and anatomical polyaxial locking plate fixation.The control group was treated with traditional "L" shape incision combined with single axial locking plate fixation.The two groups were recorded cases of operation and hospitalization time,two cases of fracture healing time,measured the day after surgery and 6 months after B,hler angle,Gissane angle,the day after surgery and 6 months after surgery,Calcaneal(calcaneal width measurements),walking function recovery time,two cases after 6 months,the excellent and good rate of ankle function of the total efficiency of two groups of patients after 6 months the incidence of complications,and do statistical analysis.Results: In the observation group,the incision healed by first intention,no incision complications and fracture nonunion.In the control group,2 cases had skin edge necrosis and incision infection occurred in 1 cases.All the patients were followed up for 12-24 months.The operation time of the observation group was 50.4±11.8min,hospitalization time was 6.1 ±1.6d,and the operation time of the control group was 56.8±12.4min,hospitalization time was 7.1±1.8d.There was significant difference between the two groups(P < 0.05).The fracture healing time of the observation group was 45.6 ± 6.7d,and the recovery time of walking function was 71.2±10.3d.The fracture healing time of the control group was 50.8±8.3d,and the recovery time of walking function was 88.4±14.2d.There was significant difference between the two groups(P < 0.05).The observation of postoperative day B?hler angle of 30.58 degrees,plus or minus 3.22 degrees,Gissane angle of 135.11 degrees,plus or minus 10.01 degrees,the control group of patients after the B?hler angle of 30.47 degrees,plus or minus 3.61 degrees,Gissane angle of 134.68 degrees,plus or minus 9.89 degrees,and there was no significant difference between the two groups(P > 0.05);the observation group of patients after 6 months B?hler angle of 29.55 degrees,plus or minus 3.14 degrees,Gissane angle of 138.44 degrees,plus or minus 12.13 degrees,the control group of patients after 6 months B,hler angle of 27.18 degrees,plus or minus 4.12 degrees,Gissane angle of 143.04 degrees,plus or minus 8.53 degrees,there were significant differences between the two groups contrast(P < 0.05).The observation of postoperative day Calcaneus width(calcaneal width measurements)was 31.67±2.37 mm,the control group of patients after the Calcaneus width measurements of 31.95±3.54 mm,there was no significant difference between the two groups(P > 0.05);measuring 6 months Calcaneus width 32.23±2.55 mm values in the observation group after surgery,the control group of patients after Calcaneus on the day of the measurement value of width was 38.67±3.42 mm,there were statistically significant differences between the two groups(P < 0.05).The observation group of patients after 6 months and the excellent rate of 83.3%,the total effective rate was 93.3%,6 months after surgery,the complication rate was 6.7%;the control group of patients after 6 months and the excellent rate of 60%,the total effective rate was 83.3%,6 months after surgery,the complication rate was 26.7%.The difference between groups was statistically significant(P < 0.05).Conclusion: The gap between the minimally invasive incision combined with sinus tarsi polyaxial locking plate in treatment of SandersⅢ type calcaneal fractures in the operation time and hospitalization time,change 6 months B?hler and Gissane angle of the healing time,postoperative fracture,the observation group 6 months after operation Calcaneus width measurements.Walking function recovery time,postoperative 6 months,the excellent and good rate of ankle function total efficiency,6 months after surgery,the incidence rate of complications were between the control group and the traditional "L" shaped plate fixation for the treatment of SandersⅢ calcaneal fracture treatment has obvious advantages compared to the traditional incision combined with calcaneal locking. |
