Objective: Through the body composition indexes with bioelectrical impedance and laboratory detection of maintenance hemodialysis patients,this research observes the changes of nutrition indicators of the maintenance hemodialysis patients,and also analyze whether the age,gender,BMI,dialysis time are the factors affecting the nutritional status of maintenance hemodialysis patients,and the correlation between body composition indexes and laboratory detection in evaluating the nutritional status of hemodialysis patients.This research is provides the basis for finding the nutritional status of maintenance hemodialysis patients in clinical application.Method: From August 2015 to September 2015,70 patients with kidney disease inurology department of Hebei general hospital were selected as the experimental group.68 cases of healthy people from physical examination center of Hebei general hospital were selected as the controls group.Inclusion criteria of experimental group:(1)Dialysis time > 3 months,dialysis frequency 3 times/week,4 hours for each time;(2)Patients have no obvious discomfort and they can take care of themselves;(3)No sodium retention and chronic heart failure;(4)No dietary intervention.Exclusion criteria of experimental group: 1)Have malignant tumor,fever or other wasting disease;2)Have serious hepatosplenomegaly;3)Have seroperitoneum;4)Have heart pacemaker,bracket or other metal in the body;5)Have serious blood system disease;6)Have blood transfusion or albumin infusion recently.Inclusion criteria of control group: No abnormalities in physical examination.Then this research collected the result of human body composition testing and laboratory indexes of the two groups.The human body composition testing included the BMI,protein content,skeletal muscle content,body fat content,body fat percentage,visceral fat area,bone mineral content,etc.Laboratory indexes included hemoglobin,albumin,triglyceride,serum total cholesterol,serum iron,total iron binding capacity,calcium,phosphorus,etc.This research analyzed the changes of nutritional status of hemodialysis patients by comparing human body composition and laboratory index between the experimental group and the control group.Meanwhile,in order to analyze the effects of the age,gander and BMI on the body composition and laboratory indexes of hemodialysis patients,the experimental group was divided into three different groups.According to the age,divided the experimental group into ?60 years old group and <60 years old group.According to the gander,divided the experimental group into male and female group.According to BMI,divided the experimental group into BMI ?24 group and BMI <24 group.This research made correlation analysis between dialysis time and body composition index and nutrition related laboratory indexes by Pearson method.It also made the correlation analysis of body composition indexes of hemodialysis in patients with skeletal muscle,protein content,fat content,albumin,percentage of body fat and blood nutrition related indexes with albumin,hemoglobin,cholesterol,triglyceride.This study analyzed whether dialysis time was the influencing factor of the nutritional status of patients and analyzed the correlation between body composition measurement indexesand laboratory indexes in assessing the nutritional status of patients.Results: 1 General condition comparison between the experimental group and the control group.There was no statistically significant difference between the two groups in age,height and gander(P>0.05).2.Comparison of nutritional indexes between the experimental group and the control group.2.1 Comparison of body composition indexes between experimental group and control group.There was no statistically significant difference between the two groupsin body fat,muscle mass,bone mineral content,body fat percentage and visceral fat area(P>0.05).Because of the indexes of protein,BMI and waist circumference in experimental group are lower than the indexes in the control group.There was statistically significant difference(P<0.05).2.2 Comparison of laboratory indexes between the experimental group and the control group.There was no statistically significant difference between the two groups in glycerin trilaurate(P>0.05).Because of the indexes of hemoglobin,albumin and serum total cholesterol in experimental group are lower than the indexes in the control group.There was statistically significant difference(P<0.05).3 Comparison of nutrition related indexes in experimental group in different age,gander and BMI 3.1 Comparison of?60 years old group and <60 years old group in experimental group.3.1.1 Comparison of body composition indexes between ?60 years old group and <60 years old group in experimental group.Because of the indexes of protein,muscle content and upper arm muscle circumference in ?60 years old group are lower than the indexes in<60 years old group.There was statistically significant difference(P<0.05).3.1.2 Comparison of laboratory indexes between ?60 years old group and <60 years old group in experimental group.Because of the indexes of iron binding capacity and serium inorganic phosphorus in ?60 years old group are lower than indexes in the <60 years old group.There was statistically significant difference(P<0.05).3.2 Comparison of male group and female group in experimental group 3.2.1 Comparison of body composition indexes between male group and female group in experimental group.Because of the indexes of height,weight,protein,muscle content,bone mineral content,ideal dry weight,upper arm muscle circumference,and bodyfat percentage in male group are higher than indexes in the female group.There was statistically significant difference(P<0.05).3.2.2 Comparison of laboratory indexes between male group and female group in experimental group There was no statistically significant difference between the laboratory indexes of two groups(P>0.05)3.3 Comparison of BMI ?24 group and BMI <24 group in experimental group 3.3.1 Comparison of body composition indexes between BMI ?24 group and BMI <24 group in experimental group Because of the indexes of height,ideal dry weight,body fat percentage,upper-arm circumference,waistline,and visceral fat area in BMI ?24 group are higher than indexes in the BMI<24 group.There was statistically significant difference(P<0.05).3.3.2 Comparison of laboratory indexes between BMI ?24 group and BMI <24 group in experimental group Because of the indexes of glycerin trilaurate in BMI ?24 group is higher than indexes in the BMI <24group.There was statistically significant difference(P<0.05).4 The correlation between the dialysis time and the nutrition related indexes in the experimental group There was no correlation between the dialysis time and hemoglobin,albumin,serum total cholesterol,glycerin trilaurate,protein,body fat,muscle and body fat percentage(P>0.05)5 The correlation between body composition measurement indexes and laboratory indexes in the experimental group Body fat percentage was negatively correlated with hemoglobin(r=-0.272)and was positively correlated with glycerin trilaurate(r=0.556)Conclusion: 1 Maintenance hemodialysis patients have malnutrition,patients most at risk for malnutrition are female,elderly and the people with low BMI.2 The nutritional status of maintenance hemodialysis patients did notchange with the duration of dialysis.3 There was correlation between body composition indexes and laboratory indexes.The measurement of human body composition has an important clinical value in the assessment of the nutritional status of patients.
