A Correlation Analysis Study On Body Compositions Inspected By Bioelectrical Impedance In Maintenance Hemodialysis Patients

Objectives:To measure fat mass and fat-free mass in maintenance hemodialysis patients with bioelectrical impedance detector. And to do a correlation analysis study on the both level of fat mass (FM) and fat-free mass (FFM) with malnutrition status, hemoglobin level, micro inflammation, plasma endotoxin level, uremic toxins level, and the incidence dialysis-related adverse of complications.Methods:We chose 60 cases of maintenance hemodialysis patients and collected their venous blood before each dialysis. We detected their serum cholesterol, serum triglycerides, serum albumin, serum urea nitrogen, serum creatinine; lymphocyte count and hemoglobin content in complete blood count (CBC), while detecting plasma endotoxin level, high sensitive C-reactive protein value (hs-CRP). When finishing dialysis, we detected FM and FFM of each patient within 10-20 minutes, using multi-frequency bioelectrical impedance detector. And we took down their weight and calculated their body mass index (BMI). According to body surface area, fat mass and fat-free mass were corrected to FM of unit surface area and FFM of unit surface area. With the Pearson linear analysis method, we did linear correlation analysis between FM of unit surface area and serum triglyceride value, serum cholesterol. And we also did linear correlation analysis between FFM of unit surface area and serum albumin, serum urea nitrogen, serum creatinine, hemoglobin, lymphocyte count, plasma endotoxin level, high sensitive CRP, as so as body mass index. According to the corrected value of FFM, these patients were divided into the low value group and the high value group. Respectively, we recorded the numbers of occurrence of adverse reactions occurred in dialysis, such as hypotension, nausea or vomiting, cramps, and count the incidence of adverse complications. Finally we did a Chi-square test between the low group and the high group.Results:1. Fat mass of unit surface area and serum triglyceride for Pearson correlation analysis: r=0.334,P=0.009<0.05.2. Fat mass of unit surface area and serum cholesterol for Pearson correlation analysis:r=0.268,P=0.039<0.05.3. Fat-free mass of unit surface area and serum albumin for Pearson correlation analysis between the two:r=0.421, P=0.001<0.05.4. Fat-free mass of unit surface area and lymphocyte counts for Pearson correlation analysis:r=-0.082, P=0.533>0.05.5. Fat-free mass of unit surface area and hemoglobin for Pearson correlation analysis:r=-0.198, P=0.149>0.05.6. Fat-free mass of unit surface area and serum endotoxin for Pearson correlation analysis between the two:r=-0.618, P=0.000<0.05.7. Fat-free mass of unit surface area and high sensitive CRP for Pearson correlation analysis:r=-0.786, P=0.000<0.05.8. Fat-free mass of unit surface area and blood serum nitrogen for Pearson correlation analysis:r=-0.107, P=0.416>0.05.9. Fat-free mass of unit surface area and serum creatinine for Pearson correlation analysis:r=0.179, P=0.171>0.05.10. Fat-free mass of unit surface area and body mass index for Pearson correlation analysis:r=0.516,P=0.000<0.05.11. In this experiment these patients, mean fat-free mass of unit surface area was 28.872kg/m2. According to this value, patients were divided into two groups, less than or equal to 28.872kg/m2 for A group (30 cases), more than 28.872kg/m2 for B group (30 cases). We recorded the incidence of adverse reactions between the two groups. During the observation of one month, we found the incidence of hypotension in A group and B group was 28.9% and 17.2% respectively, the value ofχ2 was 13.16, P<0.05; the incidence of nausea or vomiting in A group and B group was 17.8% and 13.3% respectively, the value ofχ2 was 8.96,P<O.05; the incidence of muscle cramps in A group and B group was 15.6% and 6.9% respectively, the value ofχ2 was 12.52, P<0.05. Conclusion:1. Using a bioelectrical impedance analyzer, we recorded the fat mass and fat-free mass in maintenance hemodialysis patients. The study found that the fat mass of unit surface area was associated with serum triglyceride and serum total cholesterol; the fat-free mass of unit surface area was associated with serum albumin, plasma endotoxin, high sensitivity C reactive protein and body mass index in maintenance hemodialysis patients.2. There was no correlation between the fat-free mass of unit surface and the lymphocyte count in peripheral blood. There was no correlation between the fat-free mass of unit surface and serum creatinine uremic toxins. There was no correlation between the fat-free mass of unit surface and serum urea nitrogen. And there was no correlation between the fat-free mass of unit surface and hemoglobin content in peripheral blood.3. This study found that the fat-free mass levels in maintenance hemodialysis patients were obviously associated with the incidence of dialysis-related adverse reactions such as hypotension, nausea or vomiting, muscle cramps.
