Objective Artificial total hip replacement arthroplasty which is increasingly common in clinical application, has become the first choice in the treatment of the end-stage hip joint diseases. After THA,in order to reduce the hydrops and provide against the hematoma and infection, drainage tubes are widely used. This research,by analyzing the clinical effects of the drainage tube in decreasing postoperative complications and improving the quality of patient health after the first THA,discuss the reasonability and safety of drainage tube,to bring forward reasonable suggestions for wound care. Methods The 52 selected cases of the first unilateral THA which conform with this research, were divided randomly into the drainage group and the non-drainage group respectively includes 26 patients.Observe and compare the three project indicators: 1Objective test content, such as hemoglobin before and after operation, perioperative blood losses, serum protein and the decline of HCT.2 Complications such as wound discharge, hematoma, infection and deep venous thrombosis.3 Other indicators include the cases of blood transfusion, hospital costs(exclude the pay of prosthesis),emotional scale after surgery(Hamilton Anxiety Rating Scale,HAMA)and the Charnley scales of hip joint at the 3,6 and 12 months after leaving hospital.Using statistical analysis to objectively evaluate the clinical effect and safety of the drainage tube after THA.Results 1 By comparison, serum protein, the decline of HCT and the cases of blood transfusion of the drainage group are significantly higher than those of the non-drainage group, drainage of overall incidence of complications is higher than not.the hospital costs of the non-drainage group are less than those of the drainage group, the Charnley scale of hip joint at the 12-month after leaving hospital is elevated more quickly than this scale at the three-month.The HAMA of the drainage group are obviously increased than those of the non-drainage group, these difference are statistically significant(P<0.05).2 The incidence rate of complications, such as wound discharge, hematoma, infection deep venous thrombosis, are higher in the drainage group than in the non-drainage group, but these difference are not statistically significant between two groups.hemoglobin after operation and perioperative blood losses between two groups are not indifference. The Charnley scales of hip joint at the 3,6 and 12 month after leaving hospital between two groups have no significant difference(P>0.05).Conclusion Using drainage tubes after THA dose not reduce the incidence rate of complications,on the contrary,it can increase bleeding tendency, the rate of blood transfusion and cost, causing negative emotions. The use of the drainage tube dose not promote the healing practices and the functional recovery of the hip joint. After the first THA,using the drainage tubes inside the wound has slight significance, and the wound needs further to be treated properly.
