Arthroscopic Treatment For Sore Pain Of Ankle Due To Different Causes

Object: To evaluate the clinical efficacy of arthroscopic treatment for sore pain of ankle caused by different causes.Methods: Retrospective analysis was conducted to include a total of 30 patients who undewent arthroscopic treatment for sore pain of ankle due to different causes at First Affiliated Hospital of Shihezi University from 2010 to 2015.There were 15 males and 15 females with a mean age of 48 yeas(16-76yeas).It includes 10 cases of left,19 cases of right and 1 cases of bilateral.In the process of examination,there are many cases of pain: anteriorolateral 16,anterior 5,anterioromedial 5,posterior 4.Etiology: 8cases of osteoarthritis,8 cases of acute and chronic synovitis,7 cases of traumatic arthritis,3 cases of osteoarthritis with acute and chronic synovitis,1 cases of traumatic arthritis with rheumatoid arthritis,1cases of acute and chronic synovitis with gouty arthritis,1 cases of rheumatoid arthritis,1 cases of gouty arthritis.Exclude the ankle joint gap disappeared completely;Exclude the surgical site infection of soft tissue;Exclude bleeding prone patients;Exclude high arch foot,strephenopodia and other serious deformity patients.The mean duration between the initial and treatment was 48 months(1 month to 21 yeas).All patients had routine ankle X-ray and ankle MRI,12 cases of patients with osteophytes and free body,6cases of old avulsion fracture.MRI showed 18 patients with ankle joint effusion,15 patients with soft tissue entrapment,the anterior and anterolateral of the ankle were most often swollen.For patients with simple arthroscopic synovectomy,debridement,exfoliative cartilage osteophytectomy,free body resection.Patients were followed up for 3 months and 6 months after operation respectively.The AOFAS(the American Orthopaedic Foot and Ankle Society)and VASFA Clinical Rating System and meisilin criteria for the ankle-hindfoot on ankle joint function were evaluated.Using SPSS 22 statistical software,the collected data were statistically processed.The AOFAS score before operation,3 months after operation and 6 months after operation were analyzed by paired t test.The difference of p<0.05 score was statistically significant;The patients were followed up at 3 months,months and 6 months after operation.The VASFA scores were compared by rank sum test,and the difference was statistically significant;The Meislin criterie6 months after surgery,3 months after the operation(meislin standard)to calculate the total excellent and good rate.Results: The group mean AOFAS score increased from 61.73±5.70 preoperative to 84.90±1.97 and89.53±4.66 at 3 and 6 months postoperative,respectively(P<0.05).Meislin criteria evaluation results:good excellent scores in 9 patients(30 %)at the 3 months follow-up,good excellent scores in 26 patients(86.7%)at the 6 months follow-up?VAS values were also improved at the last follow-up.Conclusions: Due to various reasons of ankle arthroscopy has good clinical curative effect of joint swelling and pain,it is worthy of popularization and application.
