Application Of Antihypertensive Drugs In Outpatient Department Of A Hospital

ObjectivesObjective to define the current status of antihypertensive drugs used in outpatients with hypertension in An Zhen Hospital of Capital Medical University,and the trend of outpatient visits for hypertension patients in Beijing,and to provide scientific basis for drug treatment and outpatient follow-up of patients with hypertension.Methods1 ? The research object was selected from our hospital outpatient information system,the subjects of this study were selected 2015.5-2015.12 patients with hypertension in Beijing medical insurance clinic in An Zhen Hospital.The determination of hypertension is based on the primary diagnosis and secondary diagnosis of outpatient information,including antihypertensive drugs within the prescribed medical information.The primary diagnostic name is referenced to the diagnostic ICD code,antihypertensive medications include all antihypertensive drugs included in the pharmacy at An Zhen Hospital during the 2015.5-2015.12.2 ? Selected subjects:Collect medical information of Beijing medical insurance patients during the period of 2015.5-2015.12 to affiliated to Beijing An Zhen Hospital of Capital Medical University,and then screen included primary diagnosis or secondary diagnosis of hypertension",and patients with antihypertensive drugs under the medical advice program,collect the information of outpatient clinic and clean and remove invalid prescription.A total of 269389 prescriptions were selected,involving a total of 34453 patients3?Access software was used to collect and classify the number of visits,the number of visits and the number of treatment programs of hypertension patients in our hospital,and screened out patient whose number of visits to our hospital >1,and the total number of treatment program was 1(or treatment program does not change),the number of visits to 14768 cases,involving the number of prescriptions for187047.The outpatient prescription of hypertension patients in each department was collected by using the perspective function of Excel software,1.follow up analysis of the use of antihypertensive drugs in various departments.4 ? In the information system of the cooperative unit(Beijing medical insurance center),The selected 14768 patients were enrolled in the outpatient service of all medical institutions in Beijing during the period of 2015.5-2015.12,describe and analyze the choice of medical institutions for the study subjects.5 ? Objective to study the clinical application of antihypertensive drugs in hypertensive patients and the different characteristics of antihypertensive drugs in different departments,according to the subjects of the An Zhen Hospital,the behavior of the patients in the clinic was divided into two groups,it can be divided into four parts: general medicine department,hypertension department,Department of Cardiology and other department.The rank characteristics of medical institutions in outpatient service were analyzed,the medical institutions will be divided into non community health service centers,community health service centers.6 ? The subjects of this study were medical insurance patients,and the initial extraction and import of data were completed by computer,without manual intervention.Results1?Through the analysis of the use of hypertension drugs,our hospital outpatient hypertension patients with a single drug use ratio was 49.41%,1.,the proportion of combined use of drugs was 50.59%.2?There were significant differences between the four departments in the combined medication plan(p<0.01).The utilization rate of combined medication in general surgery,department of Cardiology,hypertension department and other departments was 76.77%,74.31%,91.05%,76.05%,respectively,the use rate of antihypertensive drugs in hypertension clinic was higher than that of the other three departments(p<0.01),the difference was statistically significant.3?The utilization rate of the total fixed dose compound preparation in our hospital was 41.06%,mainly with the new compound preparation.The proportion of the use of the compound formula in the general department,department of Cardiology,hypertension department and other departments was 4.74%,9.65%,17.64%,5.02%.4?Our hospital outpatient antihypertensive outpatient two drug combination,the guidelines recommend the use of the ratio of 77.57%.There are differences in the use of recommended guidelines for two medications in each department(p<0.01),The proportion of general surgery,Department of Cardiology,hypertension,and other subjects were 80.47%,76.72%,76.78%,75.58%.The general medical department was more recommended than the other three groups in the two antihypertensive treatment regimen(p<0.01).The difference was statistically significant.5? Women who are older than or equal to 60 years old are more likely to choose community health service centers for medical treatment(p<0.05).ConclusionThis investigation indicates that the drug treatment program for hypertension patients in our hospital is mainly based on combined use of drugs.The use of fixed dose compound preparation is mainly based on the new compound preparation.Hypertension treatment of each department are different,and has a statistically significant difference(p<0.01),patients with hypertension outpatient two drug treatment options than the other three groups is more consistent with the guidelines recommended(p<0.01).Female,age greater than or equal to 60 years of age,hypertensive patients without complications were more likely to choose community health service the center for treatment(p<0.05).
