Objective: Retrospective analyse the value of endoscopic ultrasonography in the diagnosis of gastric stromal tumor,to explore the correlation between the age,gender,location,the internal echo,the maximum diameter and tumor risk degree,and to improve the assessment of tumor risk in endoscopic ultrasonography,providing a basis of clinical treatment for patients.Methods: Chosen the clinical data of 46 cases of gastric stromal tumor patients which was diagnosed by pathology in the affiliated hospital of Qingdao university from June2015 to October 2016,according to the risk degree method developed on 2001 the U.S.National Institutes of Health,GISTs were classified as high risk group,medium risk group,low risk group and the extremely low risk group.The extremely low risk groups which had similar clinical behavior with low risk groups,and it was difficult to distinguish from them.To facilitate the study,the extremely low risk groups were merged into the low risk groups.All 46 patients were examined by endoscopic ultrasonography before operation,Statistical methods were used to compare the relationship between tumor risk factors of gastric stromal tumors and age,sex,tumor growth site,internal echo and maximum diameter.Results: Among the 46 patients which were diagnosed by pathology,the ratio of male to female was 1.09:1,aged 20-81 years old,average aged of 57.9.The clinical manifestations were various.There were 22 asymptomatic patients,accounting for 47.8%,and 24 cases with symptoms,among them the unexplained abdominal pain,abdominal distension and early satiety were 20 cases,the black stool were 4 cases,accounting for43.5%,8.7% respectively.There were only 1 cases of abdominal mass,accounting for2.2%.Pathological information confirmed that among the 46 cases,there were 32 cases of low risk,9 cases of moderate risk and 5 cases of high-risk.By statistical analysis,the age,gender were associated with the risk of tumors had no statistical difference,P> 0.05;According to the growth site of gastric stromal tumors,it was divided into the gastric fundus,gastric body,gastric antrum.Tumor located in gastric fundus,gastric body and gastric antrum were 30,12,4 cases,accounted for 65.22%,26.09%,8.69% respectively.It can be seen from the data that the incidence of gastric stromal tumor is higher in gastric fundus and gastric body;Among the group of 46 cases,there were 21 cases of low risk, moderate in 6 cases,3 cases of high in gastric fundus;8 cases of low risk,moderate in 2cases,2 cases of high in gastric body,;3 cases of low risk,moderate in 1 cases,0 cases of high in gastric antrum.By statistical analysis,the location were associated with the risk of tumors had no statistical difference,P>0.05;According to the internal echo of gastric stromal tumors,it was divided into echo homogeneous group and heterogeneous group.In the high risk group,there were 0 cases in the echo homogeneous group,heterogeneous group in 5 cases.In the medium risk group,there were 1 case in the echo homogeneous group,heterogeneous group in 8 cases.In the low risk group,there were 13 cases in the echo homogeneous group,heterogeneous group in 19 cases.By statistical analysis,the more uneven echo,the higher the risk classification,P<0.05;The sizes of tumors were varied the smallest diameter of about 0.6 cm from the largest of about 11.0 cm measured by endoscopic ultrasonography.The diameter of low risk group was 0.6-3.5cm,averaged2.00 cm,the medium risk group was 1.5-5.5cm,averaged 3.41 cm,the high risk group was5.0-11.0cm,averaged 7.22 cm.By statistical analysis,the larger of the tumor diameter,the higher the risk classification,P<0.05.Conclusion: Endoscopic ultrasonography is an important method to detect gastric stromal tumors with high diagnostic value,its accuracy rate is 64.79%,the study showed that the risk profile of gastric stromal tumors was not associated with age,sex,and location of the tumor,but was related to the size of the lesion and internal echo.
